摘 要:目的:分析脑膜瘤实质区及瘤周水肿区的血供特点,评价DSC-MR灌注成像对良恶性脑膜瘤的鉴别诊断价值。方法:选择2017年1月—2021年1月我院经病理证实的脑膜瘤患者64例(良性32例,恶性32例),术前进行DSC-MR灌注成像分析。测量肿瘤实质区及瘤周水肿区的最大相对脑血容量和相对平均通过时间,并对结果进行统计学分析。结果:良恶性脑膜瘤实质部分相对脑血容量值与相对平均通过时间值差异无显著性(P>0.05)。恶性脑膜瘤瘤周水肿最大相对脑血容量和相对平均通过时间均高于良性脑膜瘤(P<0.05)。结论:DSC MR灌注成像对良恶性脑膜瘤的鉴别诊断具有临床价值。Objective To analyze the blood supply characteristics of meningioma parenchyma and peritumoral edema,and to evaluate the value of DSC-MR perfusion imaging in the differential diagnosis of benign and malignant meningiomas.Methods 64 patients(32 benign and 32 malignant)with meningioma confirmed by pathology in our hospital in recent 4 years were selected and analyzed by DSC MR perfusion imaging before operation.The maximum relative cerebral blood volume and relative mean transit time of tumor parenchyma and peritumoral edema area were measured,and the results were statistically analyzed.Results There was no significant difference between the relative cerebral blood volume and the relative mean transit time in the parenchyma of benign and malignant meningiomas(P>0.05).The maximum relative cerebral blood volume and relative mean transit time of peritumoral edema in malignant meningiomas were higher than those in benign meningiomas(P<0.05).Conclusion DSC MR perfusion imaging has clinical value in the differential diagnosis of benign and malignant meningiomas.
