摘 要:目的:探讨超微血管成像(SMI)下甲状腺良恶性结节鉴别诊断的价值。方法:纳入2019年6月—2020年6月于我院60例甲状腺结节患者,共115各个结节进行彩色多普勒成像(CDFI)和SMI,对比CDFI和SMI下甲状腺良恶性结节内部及周围血流分布差异,并探讨SMI下甲状腺良恶性结节血管形态特征。结果:在CDFI、cSMI、mSMI中,甲状腺良性结节与恶性结节血流分型差异无统计学意义(P>0.05)。对于血管形态特征,甲状腺良性结节多表现为周边存在环绕型血管,而恶性结节多表现为周边存在汇聚型血管(P<0.05)。mSMI较其他两种模式,良性结节中能检出更多周边环绕型血管(P<0.05),恶性结节中能检出更多周边汇聚型血管(P<0.05)。结论:SMI较CDFI能更好地显示甲状腺结节血管形态特征,在甲状腺良恶性鉴别诊断中更有价值。Objective To explore the value of superb micro-vascular imaging(SMI)in the differentiation between benign thyroid nodules and malignant ones.Methods A retrospective analysis of 60 patients(115 thyroid nodules)undergone color doppler flow imaging(CDFI)and SMI were performed,and the different types of color inside or around the thyroid nodules between CDFI and SMI were compared.At the same time,the microvascular architectures of thyroid nodules were explored.Results There was no significant difference between types of thyroid benign and malignant nodules in CDFI,cSMI and mSMI(P>0.05).Benign nodules showed complete or incomplete ring-like vessels surrounding,and malignant nodules showed converged vessels around the lesion(P<0.05).In mSMI,more ring-like vessels in benign nodules and converged vessels in malignant nodules were detected than in CDFI and cSMI(P<0.05).Conclusion Microvascular architectures on SMI maybe helpful in the differentiation of thyroid nodules.
