摘 要:目的观察肺结核病人外周血中长链非编码RNA MIR155宿主基因(LncRNAMIR155HG)的表达,研究与T细胞的关系,探讨在疾病发生中的意义。方法收集2018年1月至2019年1月河南科技大学第一附属医院就诊的肺结核(肺结核组)治疗病人126例,同时取150例体检健康者作为对照组。实时荧光定量PCR(qRT-PCR)检测外周血中LncRNA MIR155HG水平,酶联免疫吸附(ELISA)法检测外周血中T细胞指标白介素(IL)-2、干扰素(IFN-γ)、IL-4、IL-5、辅助性T细胞17A(Th-17A)水平;Pearson法分析肺结核病人外周血中LncRNA MIR155HG与T细胞指标相关性;logistic分析影响肺结核的因素;受试者工作特性曲线(ROC)分析外周血中LncRNA MIR155HG对肺结核的诊断价值。结果肺结核组和对照组外周血中LncRNA MIR155HG相对表达量OD值分别为(1.96±0.62)(1.02±0.26),与对照组相比,肺结核组外周血中LncRNA MIR155HG、IL-4、IL-5、Th-17A水平升高(P<0.05),IL-2、IFN-γ降低(P<0.05)。Pearson法分析结果显示,肺结核病人外周血中LncRNA MIR155HG与IL-2、IFN-γ呈负相关(P<0.05),与IL-4、Th-17A呈正相关(P<0.05)。logistic回归分析显示,LncRNA MIR155HG、IL-4是影响肺结核的危险因素,IFN-γ是影响肺结核的保护因素。ROC曲线显示,外周血中LncRNA MIR155HG水平预测肺结核的ROC为0.920(95%CI:0.880~0.960),截断值为1.358,其敏感性为87.3%、特异性为99.2%。结论肺结核病人外周血LncRNA MIR155HG中高表达,与T细胞指标关系密切,可在一定程度上诊断肺结核,可为早期诊断肺结核提供一定的依据。Objective with pulmonary tuberculosis,to study its relationship with T cells,and to explore its significance in the pathogenesis of pulmonary tuberculosis.Methods A total of 126 patients with pulmonary tuberculosis(tuberculosis group)treated in The First Affiliated Hospital of Henan University of Science and Technology from January 2018 to January 2019 were collected,and 150 healthy persons were taken as control group.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the level of LncRNA MIR155 HG in peripheral blood,enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of interleukin(IL)-2,interferon(IFN-γ),IL-4,IL-5 and Th-17 A in peripheral blood;Pearson method was used to analyze the correlation between LncRNA MIR155 HG and T cell index in peripheral blood of patients with pulmonary tuberculosis;Logistic analysis was used to analyzed the factors affecting tuberculosis;and the diagnostic value of LncRNA MIR 155 HG in peripheral blood for tuberculosis was analyzed by ROC.Results The OD value of the relative expression of LncRNA MIR155 HG in the peripheral blood of the tuberculosis group and the control group were 1.96±0.62,1.02±0.26,respectively.Compared with those in the control group,the levels of LncRNA MIR 155 HG,IL-4,IL-5 and Th-17 A in peripheral blood of pulmonary tuberculosis group increased(P<0.05),while the levels of IL-2 and IFN-γdecreased(P<0.05).Pearson analysis showed that LncRNA MIR155 HG in peripheral blood of patients with pulmonary tuberculosis was negatively correlated with IL-2 and IFN-γ(P<0.05),and positively correlated with IL-4 and Th-17 A(P<0.05).Logistic regression analysis showed that LncRNA MIR155 HG and IL-4 were risk factors for tuberculosis,and IFN-γwas a protective factor for tuberculosis.The ROC curve showed that the area under the ROC curve predicted by the level of LncRNA MIR 155 HG in peripheral blood was 0.920(95%CI:0.880-0.960),the truncation value was 1.358,and the sensitivity and specificity were 87.3%and 99.2%.Conclusion s MIR155 HG i
