洋参清透导邪方对糖尿病前期病人血清胰高血糖素样肽1、超敏C反应蛋白及胰岛素抵抗的影响

摘　　要：目的探讨中医洋参清透导邪方对糖尿病前期病人的治疗作用及血清胰高血糖素样肽1(GLP-1)、超敏C反应蛋白(hs-CRP)及胰岛素抵抗的影响。方法选取河南中医药大学第三附属医院初诊的140例糖尿病前期病人(2016年1月至2018年2月),采用随机区组设计分为试验组和对照组各70例,试验组采用中医洋参清透导邪方结合生活方式调整,对照组进行生活方式调整;对比两组的各项中医症候积分、腰围、体质量、空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(Fins)、血清GLP-1、hsCRP及胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI)。结果干预前,两组病人的中医证候积分对比差异无统计学意义(P>0.05);干预后,试验组的中医证候积分(13.80±4.21)分低于对照组(20.58±4.74)分(P<0.05);干预前,试验组和对照组的腰围、体质量、FPG、2 h PG、HbA1c、Fins、HOMA-IR、ISI差异无统计学意义(P>0.05);干预后,试验组的腰围(77.80±2.04)cm、FPG(5.41±0.58)mmol/L、2hPG(6.89±0.88)mmol/L、HbA1c(6.18±0.38)%、Fins(14.84±2.88)mU/L、HOMA-IR(0.63±0.28)低于对照组腰围(79.73±1.92)cm、FPG(5.82±0.50)mmol/L、2hPG(7.51±0.74)mmol/L、HbA1c(6.43±0.40)%、Fins(16.43±2.54)mU/L、HOMA-IR(0.86±0.33)(P<0.05),试验组的ISI(-4.36±0.48)高于对照组(-4.72±0.51)(P<0.05);干预前,试验组和对照组的血清GLP-1、hsCRP差异无统计学意义(P>0.05);干预后,试验组的血清hsCRP(2.16±0.74)mg/L低于对照组(3.21±0.90)mg/L(P<0.05),试验组的血清GLP-1(27.81±2.16)pg/mL高于对照组(24.74±2.43)pg/mL(P<0.05)。结论中医洋参清透导邪方结合生活方式调整能有效地降低糖尿病前期病人的血糖水平,改善胰岛素抵抗及提高胰岛素敏感度,降低血清hs-CRP及GLP-1水平。Objective To explore the therapeutic effect of traditional Chinese medicine Yangshen Qingtou Daoxiefang on pre-diabetes patients and the effects of serum GLP-1,hs-CRP and insulin resistance.Methods A total of 140 pre-diabetes patients newly diagnosed in the Third Affiliated Hospital of Henan University of Chinese Medicine from January 2016 to February 2018 were selected.They were divided into the experimental group and the control group,with 70 cases in each group.The experimental group was treated with the traditional Chinese medicine Yangshen Qingtou Daoxie combined with lifestyle adjustment,and the control group carried out lifestyle adjustment.The TCM syndrome scores,waist circumference,body weight,FPG,2 hPG,HbA1 c,Fins,serum GLP-1,hsCRP,HOMA-IR,ISI were compared between the two groups.Results ence in the scores of TCM syndromes between the two groups(P>0.05);After the intervention,the TCM syndrome score of the experimental group(13.80±4.21)was lower than that of the control group(20.58±4.74)(P<0.05);Before intervention,there was no significant difference in waist circumference,weight,FPG,2 hPG,HbA1 c,Fins,HOMA-IR,ISI between the experimental group and the control group(P>0.05);After intervention,the test group’s waist circumference(77.80±2.04)cm,FPG(5.41±0.58)mmol/L,2 hPG(6.89±0.88)mmol/L,HbA1 c(6.18±0.38)%,Fins(14.84±2.88)mU/L L.HOMA-IR(0.63±0.28)were lower than the control group’s waist circumference(79.73±1.92)cm,FPG(5.82±0.50)mmol/L,2 hPG(7.51±0.74)mmol/L,HbA1 c(6.43±0.40)%,Fins(16.43±2.54)mU/L,HOMAIR(0.86±0.33)(P<0.05).The ISI of the test group(-4.36±0.48)was higher than that of the control group(-4.72±0.51)(P<0.05);Before intervention,there was no significant difference in serum GLP-1 and hsCRP between the test group and the control group(P>0.05);After the intervention,the serum hsCRP of the test group(2.16±0.74)mg/L was lower than that of the control group(3.21±0.90)mg/L(P<0.05).The serum GLP-1 of the test group(27.81±2.16)pg/mL was higher than that of the control group(24.74±2
