超声引导下关节腔冲洗联合黄柏骨伤散贴敷治疗急性痛风性膝关节炎临床研究

摘　　要：目的:观察超声引导下关节腔冲洗联合黄柏骨伤散贴敷治疗急性痛风性膝关节炎的临床疗效和安全性。方法:70例急性痛风性膝关节炎患者随机分为观察组和对照组各35例,对照组口服塞来昔布胶囊和碳酸氢钠片,观察组在超声引导下行碳酸氢钠注射液关节腔冲洗联合黄柏骨伤散(由黄柏、大黄、侧柏叶、泽兰、薄荷、透骨草、海桐皮、蒲公英等组成)贴敷治疗,两组疗程均为2周。比较两组患者治疗前后的C反应蛋白、血沉及视觉模拟评分(VAS)、膝关节活动度(ROM)改善情况。结果:两组治疗后C反应蛋白、血沉及VAS评分、膝ROM改善情况均优于治疗前(P<0.05),且治疗2周后观察组优于对照组(P<0.05);观察组不良反应发生率低于对照组(P<0.05)。结论:超声引导下关节腔冲洗联合黄柏骨伤散贴敷治疗急性痛风性膝关节炎,能缓解疼痛、改善关节功能,不良反应少,是一种安全有效的方法。Objective:To observe the clinical efficacy and safety of ultrasound-guided joint cavity flushing com⁃bined with Huangbo Gushang San application in the treatment of acute gouty knee arthritis.Methods:70 patients with acute gouty knee arthritis were randomly divided into observation group and control group,with 35 cases in each.In the control group celecoxib capsules and sodium bicarbonate tablets were taken orally,and the sodium bicarbonate injec⁃tion in the joint cavity guided by ultrasound combined with application of Huangbo Gushang San(Phellodendron amu⁃rense,rhubarb,Platycladus orientalis,Adenophora,peppermint,Tougucao,Pittosporum bark,Dandelion,etc.)was ap⁃plied in the observation group.The treatment course is 2 weeks.The improvement of C-reactive protein,erythrocyte sedimentation rate,visual analog scale(VAS)and knee joint range of motion(ROM)were compared between the two groups before and after the treatment.Results:The C-reactive protein,erythrocyte sedimentation rate,visual analog scale(VAS)and knee ROM were improved than those before treatments in both groups(P<0.05),and the observation group was better than the control group after 2 weeks of treatments(P<0.05);the incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:Ultrasound-guided joint cavity flush⁃ing combined with Huangbo Gushang San application to treat acute gouty knee arthritis can relieve pain,improve joint function,with fewer adverse reactions.It is a safe and effective treatment method.
