皮下注射重组人生长激素治疗小儿生长激素缺乏症与特发性矮小症的临床疗效

摘　　要：目的探讨皮下注射重组人生长激素治疗小儿生长激素缺乏症与特发性矮小症的临床疗效。方法选取湘雅常德医院2018年1月—2019年4月收治的特发性矮小症患儿36例和生长激素缺乏症患儿37例。所有患儿均进行重组人生长激素皮下注射治疗。分别于治疗前后测量患儿血清胰岛素样生长因子1(IGF-1)水平、促甲状腺激素(TSH)水平、身高,计算身高标准差、生长速度,观察患儿不良反应发生情况。结果特发性矮小症治疗后IGF-1水平、TSH水平高于治疗前(P<0.05)。生长激素缺乏症患儿治疗后IGF-1水平、TSH水平高于治疗前(P<0.05)。患儿治疗后身高高于治疗前,身高标准差小于治疗前,生长速度大于治疗前(P<0.05)。特发性矮小症患儿发生暂时性血糖升高1例,不具有持续性特点,暂时性头痛1例,不良反应均自行改善。生长激素缺乏症患儿发生眼睑水肿1例,天冬氨酸氨基转移酶常出现轻微升高1例,不良反应均自行改善。结论皮下注射重组人生长激素治疗小儿生长激素缺乏症与特发性矮小症的临床疗效确切,可明显提高其IGF-1水平、TSH水平,还可有效促进患儿生长,同时安全性较高。Objective To investigate the clinical efficacy of subcutaneous injection of recombinant human growth hormone in the treatment of children with growth hormone deficiency and idiopathic short stature.Methods A total of 36 cases of children with idiopathic dwarfism and 37 children with GH deficiency were selected from January2018 to April 2019 in Xiangya Changde Hospital.All children were treated with recombinant human growth hormone subcutaneous injection.The serum IGF-1,TSH and height were measured before and after treatment,and the height standard deviation and growth rate were calculated to observe the adverse reactions.Results The levels of IGF-1 and TSH after treatment were higher than those before treatment(P<0.05).The levels of IGF-1 and TSH after treatment were higher than those before treatment(P<0.05).The height after treatment was higher than that before treatment,the standard deviation of height was lower than that before treatment,and the growth rate was higher than that before treatment(P<0.05).Transient hyperglycemia occurred in 1 case of children with idiopathic short stature,which did not have persistent characteristics.Transient headache occurred in 1 case,and the adverse reactions were improved by themselves.One case of eyelid edema and one case of slight elevation of aspartate aminotransferase were found in children with growth hormone deficiency.Conclusion Subcutaneous injection of recombinant human growth hormone in the treatment of children with growth hormone deficiency and idiopathic short stature has definite clinical effect,which can significantly improve the levels of IGF-1 and TSH,and can effectively promote the growth of children,and with high safety.
