Multiloc髓内钉与Phlios钢板在中老年肱骨近端骨折中的应用效果比较

摘　　要：目的探讨Multiloc髓内钉对中老年肱骨近端骨折的临床治疗效果。方法回顾性分析2017年1月至2020年1月北京市垂杨柳医院收治的58例中老年肱骨近端骨折患者的临床资料,根据手术方式的不同分为对照组(采用Phlios钢板内固定治疗,24例)和观察组(采用Multiloc髓内钉内固定治疗,34例)。比较两组的手术时间、术中出血量、住院时间及并发症发生情况,采用疼痛视觉模拟量表(visual analogue scale,VAS)评分、日本骨科协会评估治疗(Japanese Orthopaedic Association scores,JOA)评分及Oswestry功能障碍指数(Oswestry disability index,ODI)评价两组的临床疗效,采用简明肩关节功能测试(simple shoulder test,SST)及欧洲肩关节协会Constant评分系统评价肩关节功能。结果观察组的手术时间、术中出血量、住院时间均明显低于对照组,差异有显著性(P<0.05)。两组并发症发生率比较差异无显著性(P>0.05)。末次随访时,观察组患者的VAS、JOA及ODI评分均明显低于对照组,肩关节功能SST评分及Constant评分均明显高于对照组,差异有显著性(P<0.05)。结论中老年肱骨近端骨折采用Multiloc髓内钉内固定治疗可有效提升手术效果,改善肩关节功能,并能简化手术操作,减少不良事件的发生。Objective To investigate the clinical treatment effect of Multiloc intramedullary nail and Phlios plate on proximal humeral fractures in middle-aged and elderly patients.Method A retrospective analysis of the clinical data of 58 middle-aged and elderly patients with proximal humeral fractures admitted to Beijing Chuiyangliu Hospital from January 2017 to January 2020 were divided into control groups according to different surgical methods(treatment with Phlios plate internal fixation,24 cases)and the observation group(with Multiloc intramedullary nail internal fixation treatment,34 cases).The operation time,intraoperative blood loss,hospital stay and complications were compared between the two groups.The visual analogue scale,Japanese Orthopaedic Association scores and Oswestry disability index were used to evaluate the clinical efficacy of the two groups.The simple shoulder test and the European Shoulder Association Constant were used.The scoring system evaluates shoulder joint function.Result The operation time,intraoperative blood loss,hospitalization time of the observation group were significantly lower than those of the control group,and the difference was significant(P<0.05).At the last follow-up,the VAS,JOA and ODI scores of the observation group were significantly lower than those of the control group,and the SST score and Constant score of shoulder joint function were significantly higher than those of the control group.The difference was significant(P<0.05).Conclusion The use of Multiloc intramedullary nail internal fixation for middle-aged and elderly proximal humeral fractures can effectively improve the surgical effect,improve the function of the shoulder joint,and simplify the operation and reduce the occurrence of adverse events.
