益气升化汤联合小剂量利妥昔单抗治疗复发难治性血小板减少性紫癜的疗效观察

摘　　要：目的:探讨应用益气升化汤联合小剂量利妥昔单抗治疗复发难治性血小板减少性紫癜(Idiopathic Thrombocytopenic Purpura,ITP)的效果。方法:将100例研究对象随机分成试验组与对照组各50例,对照组单纯使用小剂量利妥昔单抗治疗,试验组在对照组的基础上加用中药益气升化汤治疗,比较两组的治疗效果。结果:试验组治疗总有效率显著高于对照组(P<0.05);治疗后两组血小板计数(Platelet,PLT)、白细胞介素-10(Interleukin-10,IL-10)均较治疗前提高,肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)较治疗前降低,试验组指标变化较对照组显著(P<0.05);两组治疗后T淋巴细胞CD4^(+)、CD4^(+)/CD8^(+),以及免疫球蛋白(Immunoglobutin,Ig)G显著升高,而CD8^(+)、IgA、IgM均显著下降,试验组指标变化幅度均较对照组显著(P<0.05)。结论:采取益气升化汤联合小剂量利妥昔单抗的中西医结合治疗复发难治性ITP的效果满意,值得推广。Objective:To explore the effects of the Yiqi Shenghua decoction(益气升化汤)plus low-dose rituximab on relapsed and refractory idiopathic thrombocytopenic purpura.Methods:100 research subjects were randomly divided into the experimental group and control group,with 50 cases in each group.The control group was treated with low-dose rituximab only,and the experimental group was treated with the Yiqi Shenghua decoction more.Results:The total efficiency in the experimental group was significantly higher than that in the control group(P<0.05).After treatment,both PLT and IL-10 were higher than before treatment,TNF-αwas lower than before treatment,and the change in the experimental group was more obvious than those in the control group(P<0.05).CD4^(+),CD4^(+)/CD8^(+)and IgG in the groups were all increased significantly after treatment,while CD8^(+),IgA and IgM were all decreased significantly(P<0.05),and the change in the experimental group was more significant(P<0.05).Conclusion:The Yiqi Shenghua decoction plus low-dose rituximab shows satisfactory results on relapsed and refractory ITP,and is worthy of promotion.
