加味龟鹿二仙汤治疗老年外伤后血虚证

摘　　要：目的:研究加味龟鹿二仙汤治疗老年人骨折术后血虚证的效果和作用机制,探索该汤剂的应用经验。方法:总结2018年1月-2019年12月手术治疗的120例老年股骨近端骨折手术患者的临床资料,其中治疗组60例应用西药联合加味龟鹿二仙汤治疗,对照组60例只采用西药治疗。对治疗前后患者血液血红蛋白(Hemoglobin,Hb)、网织红细胞计数(Reticulocyte Count,RTC)、维生素D_(3)(Vitamin D_(3),VD_(3))、碱性磷酸酶(Alkaline Phosphatase,ALP)等指标进行对照分析,同时观察对血虚症的治疗效果。结果:治疗组比对照组Hb、RTC、VD_(3)、骨钙素(Osteocalcin,OC)对比有显著性差异(P<0.05),而ALP、破骨细胞分化因子(Osteoclast Differentiation Factor,ODF)水平无显著性差异(P>0.05)。治疗组血虚证有效率明显高于对照组(P<0.05)。药物不良反应主要为轻度呕吐和腹泻,两组对比有显著性差异(P<0.05),无明显肝肾功能损害。结论:加味龟鹿二仙汤可有效治疗老年患者术后血虚证,促进骨折愈合。Objective:To study the effects and mechanism of the Guilu Erxian decoction(龟鹿二仙汤)on the blood-deficiency syndrome after fracture operation in the elderly,and to explore clinical experience of the decoction.Methods:120 cases from January 2018 to December 2019 were summarized.Among them,60 cases in the treatment group was treated with Western medicine plus the Guilu Erxian decoction,and 60 cases in the control group were treated with Western medicine only.Results:There were statistically significant deferences between the groups in Hb,RTC,VD_(3) and OC(P<0.05),and there was no statistically significant difference in the levels of ALP and ODF(P<0.05).The total efficiency in the treatment group was significantly higher than that in the control group(P<0.05).The adverse reactions of medicines were mainly mild vomiting and diarrhea,and there were a significant differences between the groups(P<0.05).There was no obviously liver and kidney function damage.Conclusion:The Guilu Erxian decoction is effective on blood-deficiency syndrome after fracture operation in the elderly,and can promote the union of the fracture.
