经皮穴位电刺激在老年骨科手术麻醉中的应用效果

摘　　要：目的探讨经皮穴位电刺激在老年骨科手术麻醉中的应用。方法选取我院2017年6月-2020年6月收治的253例行老年骨科手术的患者,随机分为观察组(n=127)和对照组(n=126),对照组采用全身麻醉,观察组在对照组基础上联合经皮穴位电刺激治疗,比较两组平均动脉压、心率、BIS值。结果 T_(1)、T_(2)时间点,两组MAP、HR水平低于T_(0)时间点,且观察组MAP、HR水平均低于对照组,差异均有统计学意义(P<0.05);T_(3)、T_(4)时间点,两组MAP、HR水平与T0时间点比较,差异无统计学意义(P>0.05),两组MAP、HR水平比较,差异无统计学意义(P>0.05);T_(1)、T_(2)时间点,两组BIS水平低于T0时间点,观察组BIS水平低于对照组,差异均有统计学意义(P<0.05);T_(3)时间点,两组BIS水平低于T0时间点,差异有统计学意义(P<0.05),但两组比较,差异无统计学意义(P>0.05);T_(4)时间点,两组BIS水平与T_(0)时间点比较,差异无统计学意义(P>0.05),两组比较,差异无统计学意义(P>0.05)。结论经皮穴位电刺激在老年骨科手术麻醉中疗效较好,可有效改善患者血流动力学波动,减轻机体应激反应,降低BIS水平,安全性高。Objective To explore the application of percutaneous electrical acupoint stimulation in the anesthesia of elderly orthopedic surgery.Methods A total of 253 elderly orthopedic surgery patients admitted to our hospital from June 2017 to June 2020 were selected and randomly divided into observation group(n=127)and control group(n=126).The control group was given general anesthesia,and the observation group was combined with percutaneous electrical acupoint stimulation on the basis of the control group.The average arterial pressure,heart rate,and BIS value of the two groups were compared.Results At T_(1) and T_(2),the MAP and HR levels of the two groups were lower than the T0 time point,and the MAP and HR levels of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05);At T_(3) and T_(4) time points,there was no statistically significant difference between the two groups of MAP and HR levels at the T_(0) time point(P>0.05).There was no statistically significant difference in the levels of MAP and HR between the two groups(P>0.05);At T_(1) and T_(2),the BIS level of the two groups was lower than the T0 time point,and the BIS level of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05);At T3 time point,the BIS level of the two groups was lower than T0 time point,the difference was statistically significant(P<0.05).However,the difference between the two groups was not statistically significant(P>0.05);At T_(4) time point,there was no statistically significant difference in BIS levels between the two groups and T_(0) time point(P>0.05).There was no statistically significant difference between the two groups(P>0.05).Conclusion Transcutaneous electrical acupoint stimulation has a good effect in anesthesia during orthopedic surgery for the elderly.It can effectively improve the patient's hemodynamic fluctuations,reduce the body's stress response,and reduce the level of BIS with high safety.
