摘 要:目的探讨经食管超声心动图(TEE)联合右心声学造影(cTTE)在观察房间隔卵圆孔未闭(PFO)中的临床应用价值。方法选取北部战区总医院自2016年1月至2018年12月收治的PFO伴偏头痛、并行介入治疗的138例患者为研究对象。根据PFO解剖分型,将患者分成简单型PFO组(n=49)和复杂型PFO组(n=89)。比较两组的隧道长度、原发隔活动度、欧氏瓣长度、分流角度、孔径大小、继发隔厚度及分流情况。结果复杂型PFO组隧道长度、原发隔活动度、欧氏瓣长度均大于简单型PFO组,差异有统计学意义(P<0.05)。复杂型PFO组分流角度小于简单型PFO组,差异有统计学意义(P<0.05)。简单型PFO组和复杂型PFO组的孔径大小、继发隔厚度差异无有统计学意义(P>0.05)。复杂型PFO组Ⅲ级、Ⅳ级分流率高于简单型PFO组,差异有统计学意义(P<0.05)。结论 TEE联合cTTE可明确PFO的解剖特征及分流程度,对于隧道长、原发隔活动度大、欧氏瓣长、分流角度小、分流等级高的PFO应足够重视。Objective To explore the application value of transesophageal echocardiography(TEE)combined with right heartcontrast transthoracic echocardiography(cTTE)in patent foramen ovale(PFO).Methods 138 patients with migraine and PFO by interventional therapy was selected from January 2016 to December 2018.According to the anatomical classification of PFO,the patients were divided into simple PFO group(n=49)and complex PFO group(n=89).Tunnel length, primary septal activity, eustachian valve, shunt angle, PFO size, thickness of secundum atrial septal shunt conditionwere compared between these two groups.Results Tunnel length, primary septal activity, eustachian valvein complex PFO group was higher than that in simple PFO group, the difference was statistically significant(P<0.05);shunt angle in complex PFO group was less than that in simple PFO group, the difference was statistically significant(P<0.05);PFO size, thickness of secundum atrial septal, there was no significant difference(P>0.05).The proportion of grade Ⅲ and Ⅳ in complex PFO group was higher than that in simple PFO group, the difference was statistically significant(P<0.05).Conclusion TEE combined with CTTE can determine the anatomical characteristics and shunt degree of PFO.
