腹膜透析相关性腹膜炎的病原菌及其耐药性分析

摘　　要：目的研究腹膜透析相关性腹膜炎的病原菌和耐药性情况。方法选取2019年8月至2021年7月本院肾内科腹透中心腹膜透析相关腹膜炎患者120例为研究对象,对腹透液标本进行细胞计数、分类及涂片染色查找病原菌。结果120例腹膜炎患者中,培养阳性96例,阳性率为80%,其中革兰阳性球菌69例,占比71.875%,以链球菌、肠球菌为主,对氯霉素、万古霉素和氨基糖苷类药物敏感;革兰阴性菌27例,占比28.125%,革兰阳性球菌多于革兰阴性菌,差异有统计学意义(P<0.05)。结论腹膜透析相关性腹膜炎患者病原菌中主要为革兰阳性球菌,对少数抗菌药物较敏感,在治疗工作中,需根据患者的实际情况选择具体药物。Objective To study the pathogenic bacteria and drug resistance of peritonitis associated with peritoneal dialysis.Methods 120 patients with peritonitis associated with peritoneal dialysis were excluded from the Peritoneal Dialysis Center of the Department of Nephrology of our hospital from August 2019 to July 2021.The cell count and classification and smear staining to find pathogens for peritoneal dialysis fluid specimen were performed.Results Among the 120 patients with peritonitis,96 cases were culture-positive,the positive rate was 80%.Among them,69 cases were Gram-positive cocci,accounting for 71.875%,streptococci and enterococci were predominant,and they were sensitive to chloramphenicol,vancomycin and aminoglycosides.27 cases of Gram-negative bacteria,accounting for 28.125%,more gram-positive cocci than Gram-negative bacteria,the difference was statistically significant(P<0.05).Conclusion The pathogenic bacteria in peritonitis associated with peritoneal dialysis patients are mainly Gram-positive cocci,which are sensitive to a few antibacterial drugs.In the actual treatment work,specific drugs need to be selected according to the actual situation of the patients.
