摘 要:目的分析甲状腺超声影像与数据报告系统(TI-RADS)联合彩色血流显像诊断甲状腺良恶性结节的效果。方法选取本院2019年1月至2020年1月接收的98例甲状腺结节患者为研究对象,均施以超声、TI-RADS、彩色血流显像检查;观察98例甲状腺结节患者超声征象,比较不同方法诊断甲状腺结节的敏感性、特异性与准确性。结果病理检查提示,98例甲状腺结节患者中良性结节76例,恶性结节22例。甲状腺恶性结节患者边界不清率(36.36%)、形态不规则率(31.82%)、纵横比≥1率(40.91%)、低回声率(81.82%)、微钙化率(31.82%)、淋巴结异常率(36.36%)显著高于甲状腺良性结节患者(6.58%、3.95%、2.63%、43.94%、43.94%、2.63%),差异均有统计学意义(P<0.05)。TI-RADS联合彩色血流显像诊断甲状腺良恶性的敏感性(96.05%)、特异性(90.91%)、准确性(94.90%)均明显高于单独TI-RADS(72.73%、81.58%、81.58%)、彩色血流显像检测(63.64%、72.37%、70.41%),差异均有统计学意义(P<0.05)。结论在甲状腺良恶性结节诊断中TI-RADS与彩色血流显像的联合应用效果较佳。Objective To analyze the effect of thyroid ultrasound and data reporting system(TI-RADS)combined with color flow imaging in the diagnosis of benign and malignant thyroid nodules.Methods 98 patients with thyroid nodule in our hospital from January 2019 to January 2020 were selected as reseach subjects and they were examined by ultrasound,TI-RADS and color blood flow imaging.The ultrasonic signs of 98 patients with thyroid nodule were observed and the sensitivity,specificity and accuracy of different diagnostic methods were compared.Results Pathological examination showed that 76 cases were benign and 22 cases were malignant.The rate of unclear border(36.36%),irregular shape(31.82%),aspect ratio≥1(40.91%),low echo rate(81.82%),microcalcification rate(31.82%),abnormal lymph node rate(36.36%)were significantly higher than those of benign thyroid nodules(6.58%,3.95%,2.63%,43.94%,43.94%,2.63%),the difference was statistically significant(P<0.05).The sensitivity(96.05%),specificity(90.91%)and accuracy(94.90%)of TI-RADS combined with color flow imaging were significantly higher than those of TIRADS alone(72.73%,81.58%,81.58%),and color flow imaging(63.64%,72.37%,70.41%),the difference was statistically significant(P<0.05).Conclusion In the diagnosis of benign and malignant thyroid nodules,the combination of TI-RADS and color blood flow imaging is better.
