摘 要:目的探讨床旁超声联合5G网络下远程会诊系统在新型冠状病毒肺炎诊疗中肺部检查中的价值。方法选取2020年2月19日~4月5日期间,对军队支援湖北省妇幼保健院光谷院区收治的1765例确诊患者中62例按照“BLUE-PLUS方案”行床旁肺部超声检查。将肺部超声图片、动态影像及测量数据等通过5G网络实时传输至绿区的超声诊断系统,与高年资超声医师一起观察记录分析患者肺部特点。结果脏层胸膜不光滑毛糙增厚者59例,B线增多者12例,肺实变者7例,含气支气管征者7例,胸腔积液者5例,未见明显异常者3例。其中脏层胸膜不光滑毛糙增厚在轻症患者、重症及危重症患者中均出现,肺实变、动态支气管充气征及胸腔积液则均出现在重症及危重症患者中,而B线增多2例出现在轻症患者,10例出现在重症及危重症患者中。病变分布方面:59例肺部超声异常患者中,55例位于PLAPS点区域,52例位于膈肌线区域,26例位于后蓝点区域,17例位于下BLUE点区域,5例位于上BLUE区域,多个区域可同时受累。结论床旁超声联合5G网络下的远程会诊系统在新型冠状病毒肺炎的诊疗中具有一定诊断意义,有助于为患者的治疗争取救治时间。Objective To explore the value of bedside ultrasonography combined with the remote consultation system under 5 G network in lung examination in the diagnosis and treatment of COVID-19. Methods Sixty-two of 1765 confirmed patients who were admitted to the Military Supported Optics Valley Branch of Maternity and Child Healthcare Hospital of Hubei Province from February19, 2020 to April 5, 2020 underwent bedside lung ultrasonography as per the "BLUE-PLUS protocol". The lung ultrasound images,dynamic images and measurement data were transmitted via 5 G network to the ultrasonic diagnostic system in the green zone. Senior sonographers together observed and recorded them to analyze lung characteristics of the patients. Results Among these patients,there were 59 patients with rough and thickened visceral pleura, 12 patients with increased B-line counts, seven patients with lung consolidation, seven patients with air bronchogram sign, five patients with pleural effusion, and three patients with no significant abnormality. Rough and thickened visceral pleura occurred in mild cases, severe cases and critical cases. Lung consolidation,dynamic air bronchogram sign and pleural effusion were found in severe cases and critical cases. Two mild cases and 10 cases had increased B-line counts. Considering the lesion distribution, among 59 cases with abnormalities in lung ultrasonography, 55 cases saw their lesion in PLAPS point, 52 cases in the diaphragmatic line, 26 cases in the posterior BLUE point, 17 cases in the lower BLUE point, and five cases in the upper BLUE point. Multiple points might be simultaneously involved. Conclusion Bedside ultrasonography combined with the remote consultation system under 5 G network has certain significance in the diagnosis of COVID-19, which helps gain time to treat patients.
