摘 要:目的观察小剂量氨茶碱联合呼吸功能训练治疗慢性阻塞性肺疾病(COPD)的临床效果及对肺功能的影响。方法选取2018年4月-2020年4月雅安职业技术学院附属医院收治的COPD患者86例,以双盲随机抽样法分为观察组和对照组,各43例。对照组予氨茶碱片治疗,观察组在对照组基础上接受呼吸功能训练。比较2组治疗效果,治疗前后肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]与动脉血气指标[氧分压(PaO2)、二氧化碳分压(PaCO2)]变化及不良反应(恶心呕吐、心悸乏力、头晕等)发生情况。结果治疗4周,观察组总有效率为95.35%,高于对照组的69.77%(χ^(2)=9.771,P=0.002);治疗4周后,2组FEV1、FVC和FEV1/FVC指标与PaO2、PaCO2水平均较治疗前改善,且观察组改善程度好于对照组(P<0.01);观察组不良反应总发生率为6.98%,对照组为4.65%,2组比较差异无统计学意义(χ^(2)=0.212,P>0.05)。结论小剂量氨茶碱联合呼吸功能训练治疗慢性阻塞性肺疾病临床效果佳,可显著改善患者的动脉血气指标水平及肺功能水平,且不良反应发生风险小,值得临床推广应用。Objective To observe the clinical effect of low dose aminophylline combined with respiratory function training in the treatment of chronic obstructive pulmonary disease(COPD)and its influence on lung function.Methods Eighty-six patients with COPD admitted to the Affiliated Hospital of Ya′an Vocational and Technical College from April 2018 to April 2020 were selected and randomly divided into observation group and control group with 43 cases in each group.The control group was treated with aminophylline tablets,and the observation group received respiratory function training on the basis of the control group.The results of the two groups were compared.The changes of pulmonary function indexes(FEV1,FVC,FEV1/FVC),arterial blood gas index(PaO2),carbon dioxide partial pressure(PaCO2)and adverse reactions(nausea,vomiting,palpitation and dizziness)were compared before and after treatment.Results After 4 weeks of treatment,the total effective rate of the observation group was 95.35%,which was higher than 69.77%of the control group(χ^(2)=9.771,P=0.002);After 4 weeks of treatment,FEV1,FVC,FEV1/FVC,PaO2 and PaCO2 levels were improved in both groups,and the improvement degree in the observation group was better than that in the control group(P<0.01);The total incidence of adverse reactions was 6.98%in the observation group and 4.65%in the control group.There was no significant difference between the two groups(χ^(2)=0.212,P>0.05).Conclusion Low dose aminophylline combined with respiratory function training in the treatment of chronic obstructive pulmonary disease has good clinical effect,can significantly improve the arterial blood gas index level and pulmonary function level of patients,and the risk of adverse reactions is small,it is worthy of clinical application.
