儿童传染性单核细胞增多症的异常淋巴细胞检测时机及早期抗病毒治疗价值探讨

摘　　要：目的观察传染性单核细胞增多症(IM)外周血淋巴细胞及异型淋巴细胞产生的高峰期,并探讨检测时机及早期抗病毒治疗的价值。方法回顾性分析80例IM患儿的临床资料,观察外周血淋巴细胞及异型淋巴细胞产生的高峰期。将80例患儿分成早期治疗组及晚期治疗组,比较2组住院时间、住院费用、退热时间、呼吸道症状消失时间、皮疹消退时间、肝脾1周内回缩情况。结果外周血淋巴细胞及异常淋巴细胞产生峰值时间为发病第5天,晚期治疗组与早期治疗组住院时间、住院费用、咽峡部炎症消退时间、肝脾1周回缩情况、气道梗阻比较,差异有统计学意义(P<0.05)。结论外周血淋巴细胞及异常淋巴细胞产生峰值的时间为发病第5天,早期抗病毒治疗可使IM患儿明显获益。Objective To observe the peak of production of peripheral blood lymphocytes and atypical lymphocytes in infectious mononucleosis(IM),and to explore the timing of test and the value of its early antiviral treatment.Methods A retrospective analysis in data of 80 children with IM was performed.The peak of production of peripheral blood lymphocytes and atypical lymphocytes were observed.These children were divided into early treatment group and late treatment group.The hospital stay,hospitalization expense,fever regression time,respiratory symptoms resolved time,rash resolution time,liver and spleen shrinkage within 1 week were compared and analyzed between the two groups.Results The peak of production of peripheral blood lymphocytes and atypical lymphocytes occurred on the 5 thday of disease attack.There were significant differences between the early treatment group and the late treatment group in terms of the hospital stay,hospitalization expense,pharyngeal isthmus inflammation subsided time,liver and spleen retraction in 1 week,and airway obstruction(P<0.05).Conclusion The peak of production of peripheral blood lymphocytes and atypical lymphocytes occur on the 5 thday of disease attack,early antiviral treatment can benefit significantly.
