摘　　要：目的探讨中性粒细胞与淋巴细胞比值(NLR)在下肢动脉硬化闭塞症(LASO)患者术后发生支架内再狭窄(ISR)的预测价值。方法根据LASO患者随访期间是否发生ISR,将92例LASO患者分为ISR组(n=24)和非ISR组(n=68)。分析比较ISR发生相关因素以及其与血浆NLR的关系。结果ISR组患者在糖尿病、高血脂、支架长度、NLR与非ISR组比较,差异有统计学意义(P<0.05)。糖尿病、高血脂、支架长度和NLR是LASO患者支架植入术后形成ISR的独立危险因素。NLR预测LASO患者发生ISR的受试者工作特征曲线(ROC)下面积(AUC)为0.736(95%CI 0.602~0.871),当最佳截断值为8.79时,敏感度是54.2%,特异度是94.1%。结论NLR是LASO患者支架植入术后形成ISR的独立危险因素,NLR对LASO患者形成ISR有较好的预测价值。Objective To investigate the predictive value of neutrophil-to-lymphocyte ratio(NLR)in patients with lower extremity arteriosclerosis obliterans(LASO)after intrastent restenosis(ISR).Methods Ninty-two LASO patients were divided into ISR group(n=24)and non-ISR group(n=68)according to the occurrence condition of ISR during the follow-up period.The factors related to ISR and its relationship with plasma NLR were analyzed and compared.Results There were significant differences in diabetes mellitus,hyperlipidemia,number and length of stents and NLR between the ISR group and the non-ISR group(P<0.05).Hyperlipidemia diabetes mellitus,stent length,and NLR were independent risk factors for ISR after stent implantation in patients with LASO.The area under the curve(AUC)of receiver operating characteristic(ROC)curve of NLR for predicting ISR in patients with LASO was 0.736(95%CI,0.602~0.871),when the optimal cut-off value was 8.79,the sensitivity was 54.2%,and the specificity was 94.1%.Conclusion NLR is an independent risk factor for the formation of ISR in patients with LASO after stent implantation,and NLR has a good predictive value for the formation of ISR in patients with LASO.
