沙库巴曲缬沙坦与培哚普利对改善慢性心力衰竭患者心室重构的对比研究

摘　　要：目的探讨沙库巴曲缬沙坦与培哚普利对改善慢性心力衰竭患者心室重构的效果。方法将115例心力衰竭患者分为A组(n=84)和B组(n=71),患者心力衰竭类型为射血分数降低性心力衰竭(HFr EF)、中间范围射血分数心力衰竭(HFmr EF)、射血分数保留性心力衰竭(HFp EF)。在常规用药基础上,A组加用沙库巴曲缬沙坦,B组加用培哚普利。分析并比较2组患者治疗前后超声心动图相关指标。结果2组左心室射血分数(LVEF)均较治疗前升高,差异有统计学意义(P<0.05);治疗6个月后,在HFr EF、Hfmr EF类型中,A组LVEF高于B组,差异有统计学意义(P<0.05)。2组左心室舒张末期内径(LVEDd)、左心房前后径(LA)、左室质量指数(LVMI)均较治疗前减小,差异有统计学意义(P<0.05)。结论沙库巴曲缬沙坦和培哚普利均对不同类型慢性心力衰竭患者具有改善左心室重构作用。Objective To investigate the efficacy of sacubitril/valsartan and perindopril in improving ventricular remodeling in patients with chronic heart failure.Methods A total of 115 heart failure patients were divided into group A(n=84)and group B(n=71).The types of heart failure in the two groups were heart failure with reduced ejection fraction(HFr EF),heart failure with midrange ejection fraction(HFmr EF),and heart failure preserved ejection fraction(HFpEF).On the basis of conventional medication,sacubitril/valsartan was added to the group A,and perindopril was added to the group B.Echocardiographic related indexes of two groups before and after treatment were analyzed and compared.Results The values of left ventricular ejection fraction(LVEF)in the two groups were significantly higher than those before treatment(P<0.05).After 6 months of treatment,the values of LVEF in patients with HFr EF and Hfmr EF types of the group A were significantly higher than those in the group B(P<0.05).Left ventricular end diastolic dimension(LVEDd),left atrial diameter(LA)and left ventricular mass index(LVMI)in the two groups were significantly decreased than those before treatment(P<0.05).Conclusion Both sacubitril/valsartan and perindopril can improve left ventricular remodeling in patients with different types of chronic heart failure.
