摘 要:目的通过研究儿茶酚抑素(Catestatin)与高血压患者血压晨峰升高的相关性,来揭示Catestatin在预测血压晨峰增加中的作用。方法门诊筛选符合入排标准的原发性高血压患者399例,根据动态血压监测结果,分为血压晨峰升高组(试验组)和血压晨峰正常组(对照组),比较2组动态血压参数,采集2组清晨血浆标本,用酶联免疫吸附法检测2组肾上腺素、去甲肾上腺素及Catestatin水平。结果2组比较,试验组24 h平均收缩压、24 h平均动脉压、日间平均收缩压、日间平均动脉压(dMAP)较对照组显著升高(P<0.05);试验组去甲肾上腺素水平[(1956.86±147.1)pg/ml]高于对照组[(1813.22±119.92)pg/ml],但差异无统计学意义(P>0.05);catestatin水平试验组[(93.35±11.45)pg/ml]显著低于对照组[(107.31±9.87)pg/ml],差异有统计学意义(P<0.05);进一步亚组分析发现血压晨峰增加组中CST/NE水平与血压晨峰增加值呈负相关(P<0.05)。结论试验组中去甲肾上腺素有升高趋势,而CST水平则显著下降,提示CST下降参与了血压晨峰的异常增加,并且这种改变较交感活性改变更加显著;进一步分析发现CST/NE与血压晨峰增加之间有更强的相关性,提示其可能成为预测血压晨峰异常的新指标。Objective To reveal the role of catestatin(CST)in predicting the increase in morning blood pressure surge(MBPS)by studying the correlation between CST and the increase in MBPS in patients with hypertension.Methods A total of 399 patients with essential hypertension who met the inclusion and exclusion criteria were screened in the Outpatient Department of our hospital.According to the results of ambulatory blood pressure monitoring,they were divided into increased MBPS group(experimental group)and normal MBPS group(control group).The parameters of ambulatory blood pressure of the two groups were compared.Early morning plasma samples of the two groups were collected,and the levels of epinephrine,norepinephrine and CST in both groups were detected by ELISA.Results The levels of 24-h mean systolic blood pressure(SBP),24-h mean arterial pressure(MAP),day-time mean systolic blood pressure(dSBP),and day-time mean arterial pressure(dMAP)in experimental group were significantly higher than those in control group(P<0.05).The noradrenaline(NE)levels in experimental group were higher than those in control group,but the difference was not significant(P>0.05).Moreover CST levels in experimental group were significantly lower than those in control group(P<0.05).Subgroup analysis further revealed that the CST/NE levels in MBPS increase group were negatively correlated with the increased levels of MBPS(P<0.05).Conclusion The NE levels in the experimental group have increase trend,while CST levels are decreased significantly,which suggests that the decrease in CST is involved in the abnormal increase in the MBPS,and the changes were more significant,as compared with sympathetic activity changes.Moreover there is a significant correlation between CST/NE and increase of MBPS,which suggests that it might become a new index to predict the abnormality of MBPS.
