无缝隙管理在盐酸班布特罗治疗小儿哮喘中的应用及对呼吸动力学指标的影响

摘　　要：目的探讨无缝隙护理在盐酸班布特罗治疗小儿哮喘中的应用及对小儿呼吸动力学指标的影响。方法借助数字双盲法将2018年1月至2020年1月接受盐酸班布特罗治疗的小儿哮喘患儿80例分为对照组与研究组,每组40例。治疗期间对照组给予常规护理,研究组进行无缝隙护理,比较护理前后患儿肺功能、免疫功能、住院时间、症状改善时间、护理满意情况及呼吸动力学情况。结果2组患儿护理前肺功能指标、免疫功能指标、呼吸动力学指标差异无统计学意义(P>0.05),护理后研究组患儿肺功能指标FEV1、FVC%perd、FEV1%perd改善情况均优于对照组(P<0.05),研究组患儿住院时间、症状改善时间、护理满意情况均优于对照组(P<0.05),研究组免疫功能指标CD+3水平、CD+4水平、CD+8水平均高于对照组(P<0.05),而CD+4/CD+8水平低于对照组(P<0.05),呼吸动力学指标WOB、Raw、PIP水平均低于对照组(P<0.05),而Cdyn水平高于对照组(P<0.05)。结论小儿哮喘患儿接受盐酸班布特罗治疗期间行无缝隙护理干预,可有效改善患儿肺功能与呼吸功能,提高机体免疫力,利于增强治疗效果,该模式具有较高临床应用价值。Objective To investigate the application of seamless nursing in infantile asthma treated by bambuterol hydrochloride and its effects on respiratory dynamic indexes of patients.Methods A total of 80 children with asthma who were treated by bambuterol hydrochloride in our hospital from January 2018 to January 2020 were enrolled in the study,who were divided into control group and research group according to digital double-blind method,with 40 cases in each group.The patients in control group were treated by routine nursing,however,the patients in research group were treated by seamless nursing.The pulmonary function,immune function,hospital stay,symptom improvement time,nursing satisfaction degree and respiratory dynamics indexes before and after nursing were observed and compared between the two grousp.Results Before nursing there were no significant differences in lung function indexes,immune function indexes,respiratory dynamics indexes between the two groups(P>0.05).After nursing,the lung function indexes including FEV1 and FVC%perd,FEV1%perd in research group were superior to those in control group(P<0.05),moreover the hospital sday,symptoms improvement time,nursing satisfaction degree and immune function indexes including CD+3,CD+4,CD+8 were superior to those in control group(P<0.05),however,the levels of CD+4/CD+8,and the levels of respiratory dynamics indexes including WOB,Raw and PIP were significantly lower than those in control group(P<0.05),but the Cdyn levels were significantly higher than those in control group(P<0.05).Conclusion The application of seamless nursing in treatment of infantile asthma by bambuterol hydrochloride can effectively improve the pulmonary function and respiratory function of patients,improve the body immunity,which is helpful to enhance the therapeutic effects,therefor,it is of higher clinical application value.
