摘  要:目的 分析深圳市南山区秋季腹泻和健康人群气单胞菌的感染现状、分型及耐药特征。方法 采集粪便标本,分离气单胞菌,用荧光PCR、VITEK Ⅱ Compact、16S rRNA对可疑菌进行种属鉴定,用脉冲场凝胶电泳进行分型,用纸片扩散法进行药敏试验。结果 2019年9月-11月南山区腹泻和健康人群的气单胞菌分离率分别为6. 2%(8/129)和7. 3%(11/150),两者差异无统计学意义(χ^(2)=0. 140,P> 0. 05)。19株气单胞菌中,有3株温和气单胞菌,2株维氏气单胞菌,14株嗜水气单胞或豚鼠气单胞菌。除3株菌重复2次酶切均无条带外,其余16株菌可以获得15个PFGE带型,呈多态性。气单胞菌对头孢西丁的耐药率最高(26. 3%,5/19),其次为亚胺培南(21. 1%,4/19)和复方新诺明(10. 5%,2/19)。结论 临床上分离到气单胞菌时应考虑其正常携带的情况,气单胞菌株在腹泻和健康人群中呈多态化分布。临床上使用头孢西丁和亚胺培南进行气单胞菌治疗时应慎重。Objective To analyze the prevalence,typing characteristics and antimicrobial susceptibility of Aeromonas in diarrhea and healthy people in Nanshan District,Shenzhen.Methods Fecal specimens were collected and Aeromonas were isolated.Fluorescent PCR,biochemical test and 16S rRNA were carried to identify the strains.Moreover,pulsed-field gel electrophoresis(PFGE)was performed,and antimicrobial susceptibility was determined by disk diffusion method.Results During September 2019-November 2019,the prevalence of Aeromonas isolated from diarrhea people and healthy people were 6.2%(8/129)and 7.3%(11/150),respectively.However,there was no statistical significance on the difference between diarrhea and healthy people(χ^(2)=0.140,P>0.05).Among the 19 strains,3 strains of Aeromonas sobria,2 strains of Aeromonas veronii,and 14 strains of Aeromonas hydrophila or Aeromonas caviae were identified.Except for 3 strains which had no bands after repeated experiments,15 PFGE patterns were obtained from 16 strains.Aeromonas had the highest resistance rate to cefoxitin(26.3%,5/19),followed by imipenem(21.1%,4/19),and sulfamethoxazole(10.5%,2/19).Conclusion When Aeromonas is isolated clinically,its normal carrying condition should be considered.Aeromonas strains show polymorphic distribution in diarrhea and healthy people.Cefoxitin and imipenem should be cautiously used in clinical treatment of Aeromonas.
