摘　　要：目的了解山东省各医疗机构即时检验(point of care testing,POCT)便携式血糖仪管理体系建立与运行现状,为便携式血糖仪标准化同质化管理提供依据。方法采用横断面调查法,对山东省112家医院540名护理人员进行"POCT便携式血糖仪管理体系建立与应用现状调查问卷"。结果被调查医院66所有管理委员会、54所有协调员、74所有科室管理员;对操作者培训、授权、与生化仪血糖比对工作主管部门各家医院不一致。委员会职责落实评价总分(59.66±12.56)分,设备管理、培训授权落实、质控管理三个维度得分:(14.27±2.81)分、(21.49±5.38)分、(19.98±2.39)分。存在主要问题有:委员会岗位设置不全、职责不明确,室内质控及室间质评执行频次少、便携式血糖仪智能化程度低。三级医院岗位设置和职责落实评价得分好于一、二级医院,经χ^(2)检验和t检验,P<0.01或P<0.05,差异有统计学意义。结论各医疗机构均需完善委员会建立,加强岗位职责落实,尤其是一二级医院。呼吁国家卫生管理部门制订POCT便携式血糖仪管理体系建设标准,医院重点加强质量控制管理,加强POCT便携式血糖仪专项培训,提高POCT便携式血糖仪管理水平。Objective To understand the current situation of the establishment and operation of POCT portable blood glucose meter management system in medical institutions in Shandong Province,and to provide the basis for standardized and homogeneous management of blood glucose meter.Methods Using cross-sectional questionnaire survey,540 nurses from 112 hospitals in Shandong Province were investigated by"Questionnaire on the establishment and application of POCT portable blood glucose meter management system".Results Among invested hospitals,management committees were established in 66 hospitals,54 hospitals had coordinators,74 hospitals had control administrator.The operator’s training,authorization,and portable blood glucose comparison with the biochemical meter were inconsistent in each hospital.The total score of the committee responsibility implementation evaluation was(59.66±12.56)score,and the score of equipment management,training authorization implementation and quality control management was(14.27±2.81)score,(21.49±5.38)score and(19.98±2.39)score.The main problems were:incomplete job setting of committees,unclear responsibilities,low frequency of implementation of indoor quality control and inter-room quality evaluation,and low intelligence of portable blood glucose meters.The evaluation scores of the job setting and responsibility implementation of the tertiary hospitals were better than those of the first and second-level hospitals.Afterχ^(2) test and t test,P<0.01 or P<0.05,the difference was statistically significant.Conclusion All medical institutions need to improve the establishment of the committee and strengthen the implementation of post responsibilities,especially the primary and secondary hospitals.We call on the national health management department to formulate the construction standard of POCT portable blood glucose meter management system.Focus on quality control,strengthen the special training of POCT portable blood glucose meter,and improve the management level of POCT portable blood glucos
