血清白细胞介素-18评估急性呼吸窘迫综合征预后的临床价值

摘　　要：目的探讨血清白细胞介素-18(IL-18)评估急性呼吸窘迫综合征(ARDS)预后的临床价值。方法选取在ICU收治的120例ARDS患者,采用双抗体夹心酶联免疫吸附试验(ELISA)检测血清IL-18的表达,根据检测结果分为高IL-8组和低IL-8组。治疗后对患者行2个月以上的随访,分为生存组和死亡组。用受试者工作特征(ROC)曲线分析血清IL-18水平对ARDS患者预后的预测价值。结果死亡组患者血清IL-18水平高于存活组(P<0.05)。在ROC曲线上,IL-18作为预后预测因子的敏感度和特异度在临界值为5.9 pg/mL时分别是89%和84%,ROC曲线下面积为0.85(P<0.05)。结论以5.9 pg/mL作为判断的临界值,血清IL-18水平可用于评估ARDS患者的预后。Objective To investigate the clinical value of serum interleukin-18(IL-18)in evaluating the prognosis of acute respiratory distress syndrome(ARDS).Methods A total of 120 ARDS patients admitted to ICU were selected.The expression of IL-18 in serum was detected by double antibody sandwich enzyme-linked immunosorbent assay(ELISA),and the patients were divided into high IL-18 group and low IL-18 group according to the results of detection.After treatment,the patients were followed up for more than two months.The patients were eventually divided into survival group and death group.The prognosis of ARDS patients was analyzed by ROC curve.Results The serum IL-18 level in the dead group was significantly higher than that in the survival group(P<0.05).On the ROC curve,the sensitivity and specificity of IL-18 as a prognostic predictor were 89%and 84%,respectively,at the critical value of 5.9 pg/mL;and the area under the curve was 0.85(P<0.05).Conclusion Serum IL-18 can be used to evaluate the prognosis of ARDS patients with 5.9 pg/mL as the critical value of survival.
