宫颈癌根治术后病人年龄与尿潴留发生率的关系研究

摘　　要：目的:探讨宫颈癌根治术后病人的年龄与尿潴留发生率的关系。方法:回顾性分析深圳市第二人民医院2017年3月―2020年3月收治的168例宫颈癌根治性手术病人的临床资料,记录病人导尿管持续时间和术后尿潴留发生率,并分析尿潴留发生的危险因素。结果:102例病人年龄(51.05±9.17)岁,尿潴留发生率为53.92%(55/102)。完全调整的二项Logistic回归结果显示,调整混杂因素后,年龄与尿潴留发生风险呈正相关[OR=1.21,95%CI(1.09,1.35),P=0.001]。在年龄和尿潴留之间检测到曲线关系,其拐点为54岁。拐点左侧和右侧的效应量OR值分别为0.99[95%CI(0.90,1.10)]和1.84[95%CI(1.24,2.74)]。通过亚组分析发现,留置导管越长,年龄对尿潴留发生率的影响越大。结论:年龄是宫颈癌根治术后病人发生尿潴留的独立危险因素。年龄和尿潴留呈曲线关系,54岁是拐点,临床应更加关注长期留置尿管且大于54岁的病人。Objective:To explore the relationship between age and occurrence of urinary retention(PUR)in patients after radical operation for cervical cancer.Methods:The clinical data of 168 patients undergoing radical surgery for cervical cancer from March 2017 to March 2020 in Shenzhen Second People's Hospital were retrospectively analyzed.Duration of catheters and incidence of PUR were recorded,and risk factors for PUR were analyzed.Results:The average age of the 102 participants was 51.05±9.17,and occurrence of PUR was 53.92%(55/102).Fully adjusted binomial logistic regression results showed that,after adjusting for confounding factors,age was positively correlated with risk of urinary retention(OR=1.21,95%CI(1.09,1.35),P=0.001).A curvilinear relationship was detected between age and PUR,with an inflection point of 54 years.The influence size on the left and right of the inflection point were OR=0.99[95%CI(0.90,1.10)],OR=1.84[95%CI(1.24,2.74)],respectively.Through subgroup analysis,the longer the time of indwelling catheter was,the greater the effect of age was.Conclusions:Age was an independent risk factor for PUR in patients after radical operation for cervical cancer.There was a curvilinear relationship between age and PUR.54-year-old was the inflection point.In clinical practice,more attention should be paid to patients with long-term indwelling catheters and aged above 54-year-old.
