摘　　要：目的探讨米非司酮联合左炔诺孕酮宫内缓释系统(LNG-IUS)对子宫肌瘤患者单核细胞趋化因子-1(MCP-1)表达水平的影响。方法选取医院2018年9月至2020年6月收治的子宫肌瘤患者154例,按随机数字表法将患者分为宫内缓释组与联合治疗组,各77例。两组患者均采用LNG-IUS治疗,联合治疗组患者加用米非司酮治疗,均持续治疗6个月。结果与宫内缓释组相比,联合治疗组患者的子宫内膜厚度、子宫体积及肌瘤体积均显著缩小,雌二醇、孕激素、促卵泡生成素、血管内皮生长因子、MCP-1水平,以及视觉模拟评分法(VAS)评分均显著降低,血红蛋白水平显著升高(P<0.05);联合治疗组的总有效率为97.40%,显著高于宫内缓释组的88.31%(P<0.05);宫内缓释组与联合治疗组的不良反应发生率相当(6.49%比7.79%,P>0.05)。结论米非司酮联合LNG-IUS治疗子宫肌瘤疗效较好,能有效改善临床症状、缓解病情,并有效抑制病情进展,降低MCP-1的表达水平,改善预后,且安全性较高。Objective To investigate the effect of mifepristone combined with levonorgestrel-releasing intrauterine system(LNG-IUS)on the level of monocyte chemoattractant factor-1(MCP-1)in patients with uterine myoma.Methods A total of 154 patients with uterine myoma admitted to our hospital from September 2018 to June 2020 were selected and divided into the LNG-IUS group and the treatment group according to the radom number table method,77 cases in each group.The patients in the two groups were treated with LNG-IUS,on this basis,the patients in the combined group were treated with mifepristone.Both groups were continuously treated for six months.Results Compared with those in the LNG-IUS group,the endometrial thickness,uterine volume and myoma volume in the combined group were significantly reduced,the estradiol,progesterone,follicle-stimulating hormone,vascular endothelial growth factor(VEGF),MCP-1 and visual analogue scale(VAS)score in the combined group were significantly decreased,while the level of hemoglobin in the combined group was significantly increased(P<0.05).The total effective rate of the combined group was 97.40%,which was significantly higher than 88.31% of the LNG-IUS group(P<0.05).The incidence of adverse reactions in LNG-IUS group was similar to that in the combined treatment group(6.49%vs.7.79%,P>0.05).Conclusion Mifepristone combined with LNG-IUS is effective and safe in the treatment of uterine myoma,which can effectively improve the patients’symptoms,alleviate the patients’condition,effectively inhibit the progress of the disease,reduce the expression level of MCP-1,and improve the prognosis of patients.
