盐酸米诺环素联合Vitapex根充糊剂治疗乳牙瘘管型根尖周炎临床观察

摘　　要：目的探讨盐酸米诺环素(商品名派力奥)联合Vitapex根充糊剂治疗乳牙瘘管型根尖周炎的临床疗效。方法选取北京市东城区妇幼保健院2019年1月至12月收治的乳牙瘘管型根尖周炎患儿100例,按抽签法分为对照组和观察组,各50例。两组患儿均予Vitapex根充糊剂治疗,观察组患儿加用派力奥。结果观察组的总有效率为96.00%,显著高于对照组的88.00%(P<0.05);治疗后1周和治疗后1个月,观察组患儿的疼痛视觉模拟评分法(VAS)评分均显著低于对照组(P<0.05);治疗后1周,观察组患儿的龈沟液炎性因子(白细胞介素8、肿瘤坏死因子-α)水平均显著低于对照组(P<0.05);观察组和对照组不良反应发生率相当(8.00%比4.00%,P>0.05)。结论盐酸米诺环素联合Vitapex根充糊剂治疗乳牙瘘管型根尖周炎疗效较好,能促进乳牙瘘管愈合,缓解疼痛,减轻炎性反应,且安全性较好。Objective To investigate the clinical efficacy of minocycline hydrochloride(trade name:Periocline)combined with Vitapex paste in the treatment of deciduous teeth with fistula periapical periodontitis.Methods A total of 100 children with deciduous teeth with fistula periapical periodontitis admitted to the Beijing Dongcheng District Maternal and Child Health Hospital from January to December 2019 were selected and divided into the control group and the observation group by lottery method,50 cases in each group.The children in the two groups were treated with Vitapex paste for root canal filling,on this basis,the patients in the observation group were treated with Periocline.Results The total effective rate of the observation group was 96.00%,which was significantly higher than 88.00% of the control group(P<0.05).One week and one month after treatment,the visual analogue scale(VAS)scores in the observation group were significantly lower than those in the control group(P<0.05).One week after treatment,the levels of inflammatory factors[interleukin-8(IL-8)and tumor necrosis factor(TNF)-α]in gingival crevicular fluid in the observation group were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(8.00%vs.4.00%,P>0.05).Conclusion Minocycline hydrochloride combined with Vitapex paste is effective and safe in the treatment of deciduous teeth with fistula periapical periodontitis,which can promote the healing of deciduous teeth fistula,relieve pain and inflammatory reaction.
