摘 要:目的:探究骨髓间充质干细胞(BMSCs)对大鼠骨质疏松股骨干骨折早期愈合的影响。方法:骨质疏松大鼠股骨干骨折模型随机平分为三组,模型组、尼尔雌醇组与BMSCs组。模型组给予灌胃生理盐水,尼尔雌醇组给予灌胃尼尔雌醇,BMSCs组经尾静脉注入第三代BMSCs。两组都治疗观察4周,记录股骨干骨折早期愈合情况。结果:尼尔雌醇组与BMSCs组的股骨干、股骨颈骨密度高于模型组(均P<0.05),BMSCs组高于尼尔雌醇组(P<0.05)。尼尔雌醇组与BMSCs组的血清碱性磷酸酶(ALP)含量低于模型组,BMSCs组低于尼尔雌醇组(均P<0.05)。尼尔雌醇组与BMSCs组的股骨干最大应力、最大承载力高于模型组(均P<0.05),BMSCs组高于尼尔雌醇组(均P<0.05)。尼尔雌醇组与BMSCs组的股骨干Notch1、B淋巴细胞瘤-2基因(Bcl-2)蛋白相对表达水平低于模型组(P<0.05),BMSCs组低于尼尔雌醇组(P<0.05)。结论:BMSCs在骨质疏松大鼠股骨干骨折中的应用能促进早期愈合,可抑制Notch1、Bcl-2表达,有利于降低血清ALP水平与提高骨密度,改善股骨干的生物力学状况。Objective:To explore the effect of bone mesenchymalstem cells(BMSCs)on the early healing of femoral shaft fractures in osteoporotic rats.Methods:The femoral shaft fracture model of osteoporotic rats(n=51)were equally randomly divided into model group,nylestriol group and BMSCs group.The model group was given intragastric normal saline,and the nilestriol group was given intragastric nilestriol,and the BMSCs group was injected with third-generation BMSCs through the tail vein.Both groups were treated and observed for 4 weeks,and the early healing of femoral shaft fractures were recorded.Results:The bone mineral density of the femoral shaft and neck in the nilestriol group and the BMSCs group were higher than that of the model group,and the BMSCs group were higher than the nilestriol group(all P<0.05).The level of serum alkaline phosphatase(ALP)in the nilestriol group and the BMSCs group was lower than that of the model group,and the BMSCs group was lower than the nilestriol group(all P<0.05).The maximum stress and maximum bearing capacity of the femoral shaft in the nilestriol group and the BMSCs group were higher than those of the model group,and the BMSCs group was higher than the nilestriol group(all P<0.05).The relative expression levels of Notch1 and B-cell lymphoma-2 gene(Bcl-2)proteins in the nilestriol group and the BMSCs group were lower than those of the model group and the BMSCs group was lower than the nilestriol group(all P<0.05).Conclusion:The application of BMSCs in femoral shaft fractures of osteoporotic rats can promote early healing,inhibit the expression of Notch1 and Bcl-2,reduce serum ALP level,increase bone density,and improve the biomechanical condition of femoral shaft.
