摘　　要：目的探讨前方小切口手指引导复位结合PFNA治疗难复性股骨粗隆间骨折的疗效。方法30例难复性股骨粗隆间骨折患者随机分为两组,观察组采用前方小切口手指引导复位结合PFNA治疗,对照组采用器械复位结合PFNA治疗,比较两组的治疗效果。结果观察组的透视次数、手术时间、术中出血量、术后下地时间以及住院时间均显著低于对照组(P<0.05);两组的骨折愈合时间比较,差异无统计学意义(P>0.05)。观察组的并发症发生率为6.67%,显著低于对照组的26.67%(P<0.05)。结论前方小切口手指引导复位结合PFNA内固定治疗难复性股骨粗隆间骨折,可达到满意的复位固定效果,减少并发症。Objective To explore the efficacy of small anterior incision finger guided reduction combined with PFNA in the treatment of refractory intertrochanteric fracture of femur.Methods 30 patients with refractory intertrochanteric fracture of femur were randomly divided into two groups.The observation group was treated with small anterior incision finger guided reduction combined with PFNA,and the control group was treated with device reduction combined with PFNA.The curative effects of two groups were compared.Results The number of fluoroscopy,operation time,intraoperative blood loss amount,postoperative ambulation time and hospitalization time of observation group were significantly lower than those of control group(P<0.05);No significant difference was found in the fracture healing time between the two groups(P>0.05).The incidence of complications of observation group was 6.67%,significantly lower than 26.67%of control group(P<0.05).Conclusions Small anterior incision finger guided reduction combined with PFNA in the treatment of refractory intertrochanteric fracture of femur can achieve satisfactory reduction and fixation effect and reduce complications.
