Osteotomy of Medial Malleolus or Lateral Malleolus for the Treatment of Talus Body Fracture Complicated with Dislocation of Ankle Joint and Subtalus Joint

摘　　要：目的探讨内踝或外踝截骨手术治疗距骨体骨折并踝关节、距下关节脱位的效果和安全性。方法40例距骨体骨折并踝关节、距下关节脱位患者随机分为两组各20例,对照组采用常规手术治疗,研究组采用内踝或外踝截骨手术治疗,比较两组的手术效果以及并发症。结果研究组的手术时长、住院时长、骨折愈合时间均显著短于对照组,VAS评分显著低于对照组,足部功能评分显著高于对照组(P<0.05)。研究组的并发症发生率为5.00%,显著低于对照组的30.00%(P<0.05)。结论内踝或外踝截骨手术治疗距骨体骨折并踝关节、距下关节脱位患者能有效改善足部功能,缩短骨折愈合时间,改善预后,安全性高。Objective To explore the effect and safety of osteotomy of medial malleolus or lateral malleolus for the treatment of talus body fracture complicated with dislocation of ankle joint and subtalus joint.Methods 40 patients with talus body fracture complicated with dislocation of ankle joint and subtalus joint were randomly divided into two groups,with 20 cases in each group.The control group was treated with conventional surgery,and the study group was treated with osteotomy of medial malleolus or lateral malleolus.The surgical effects and complications of two groups were compared.Results The operation time,hospitalization time and fracture healing time of study group were significantly shorter than those of control group,the VAS score was significantly lower than that of control group,and the foot function score was significantly higher than that of control group(P<0.05).The incidence of complications of study group was 5.00%,significantly lower than 30.00%of control group(P<0.05).Conclusions Osteotomy of medial malleolus or lateral malleolus for the treatment of patients with talus body fracture complicated with dislocation of ankle joint and subtalus joint can effectively improve foot function,shorten fracture healing time,and improve prognosis,with high safety.
