心脏起搏器置入术后分阶段改变患者卧床体位对并发症及舒适度的影响

摘　　要：【目的】通过分阶段改变心脏永久起搏器置入术后患者的卧床体位,观察电极脱位、囊袋血肿、卧床相关并发症的发生情况以及患者舒适度情况。【方法】收集首次行心脏双腔起搏器置入术患者100例作为研究对象,随机分为对照组和观察组,每组各50例。对照组术后平卧,72 h后下地活动;观察组术后第1个24 h内取平卧位,第2个24 h内可将床头摇高至45°,第3个24 h内可将床头摇高至70°,卧床满72 h后可下床活动。分析两组患者电极脱位、囊袋血肿、尿潴留、腹胀、腰痛的发生情况以及患者舒适度情况。【结果】所有患者未发生电极脱位和囊袋血肿。两组患者比较,尿潴留的发生率无统计学差异(P>0.05)。观察组患者腹胀发生率低于对照组(P<0.05),腰痛评分低于对照组(P<0.05),舒适度评分高于对照组(P<0.05)。【结果】心脏永久起搏器置入术后分阶段改变患者卧床体位,不增加电极脱位、囊袋血肿的发生,有利于降低卧床相关并发症,有利于提高患者的舒适度。【Objective】To observe the occurrence of the displacement of the lead, pocket hematoma, bed-related complications and comfortable sensation in patients after cardiac pacemaker implantation by grading position change in bed.【Methods】One hundred patients with dual-chamber cardiac pacemaker implantation for the first time were selected as research subjects. All the patients were randomly divided into control group(50 cases) and observation group(50 cases). The patients in the control group stayed in the supine position during 72 h after operation. The patients in the observation group stayed in the supine position during the first 24 h after operation, who could elevate the head of the bed to an angle of 45° during the second 24 h and enhance the angle to 70° during the third 24 h and get out of bed and do activities after the third 24 h. The occurrence of the displacement of the lead, pocket hematoma,urinary retention, abdominal distension, lumbago and comfortable sensation were analyzed between the two groups.【Results】There were no displacement of the lead and pocket hematoma in both groups. There was no significant difference in the incidence of urinary retention between the two groups(P>0.05). The incidence of abdominal distension and lumbago scores in observation group were lower than those in the control group(P<0.05). The scores of comfortable sensation in the observation group were more than that in the observation group(P<0.05).【Conclusion】Grading position change in bed after cardiac pacemaker implantation doesn’t enhance the incidence of the displacement of the lead and pocket hematoma. Moreover, it can reduce the incidence of bed-related complications and is conducive to the improvement of the patient’s comfortable sensation.
