尘螨特异性皮下免疫治疗尘螨过敏性哮喘伴变应性鼻炎患儿的预后分析

摘　　要：目的探讨尘螨特异性皮下免疫治疗尘螨过敏性哮喘伴变应性鼻炎患儿的预后。方法回顾性分析2015年1月—2017年12月中山市博爱医院收治的行尘螨特异性皮下免疫治疗的78例尘螨过敏性哮喘伴变应性鼻炎患儿临床资料,患儿均接受标准化安脱达皮下脱敏治疗。比较患儿治疗前和治疗后的F EV1%、FVC%和FEV1%/FVC%等肺功能指标、哮喘症状评分、鼻炎症状评分、免疫因子、T淋巴细胞亚群指标等。结果与治疗前比较,患儿治疗后的FEV1%、FVC%和F EV1%/FVC%等肺功能指标,CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均升高;患儿治疗后的哮喘症状评分、鼻炎症状评分、CD_(8)^(+)水平则均降低,差异有统计学意义(P<0.05)。结论尘螨特异性皮下免疫治疗尘螨过敏性哮喘伴变应性鼻炎患儿可改善免疫情况、肺功能、哮喘症状和鼻炎症状,值得应用。Objective To investigate the prognosis of dust mite specific subcutaneous immunotherapy in children with dust mite allergic asthma and allergic rhinitis.Methods The clinical data of 78 children with dust mite allergic asthma and allergic rhinitis who were treated with dust mite specific subcutaneous immunotherapy from January 2015 to December 2017 were retrospectively analyzed.All the children received standardized subcutaneous desensitization treatment with Antodah.The lung function indexes,such as FEV1%,FVC% and FEV1/FVC%,asthma symptom score,rhinitis symptom score,immune factors,T lymphocyte subsets and inflammation indexes were compared before and after treatment.Results Compared with the indexes before treatment,the children's lung function indexes,such as FEV1%,FVC%and FEV1/FVC%,CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)levels significantly increased after treatment,while the children's asthma symptoms score,rhinitis symptom score,CD_(8)^(+)level significantly decreased(P<0.05).Conclusion The specific subcutaneous immunotherapy of dust mite can improve immune,lung function,asthma symptoms and rhinitis symptoms for children with dust mite allergic asthma and allergic rhinitis,which is worthy of application.
