摘　　要：目的:探讨超声造影鉴别诊断结核性淋巴结炎与转移性淋巴结的价值。方法:回顾性分析在我院就诊的124例淋巴结肿大病人(结核组51例,转移组73例)的检查资料,分析比较二者超声造影声像图特征。结果:动脉期结核组患者主要表现为离心性灌注型和无灌注型,转移组患者主要表现为向心性灌注型和混合性灌注型,差异均有统计学意义(P<0.05);实质期两组患者均主要表现为不均匀增强型;两组患者灌注缺损区比较,形态、边界及灌注特征的差异均有统计学意义(P<0.05)。结论:超声造影在鉴别诊断结核性淋巴结炎和转移性淋巴结具有较高的应用价值。Objective: To investigate the value of contrast-enhanced ultrasound in the differential diagnosis of tuberculous lymphadenitis and metastatic lymph nodes. Methods: The examination data of 124 patients with lymph node enlargement(51 cases in tuberculosis group and 73 cases in metastasis group) admitted to our hospital was retrospectively analyzed, and the sonographic features of the two groups were analyzed and compared. Results: The patients in the arterial tuberculosis group were mainly of eccentric and non-perfusion types, while the patients in the metastasis group were mainly of centripetal and mixed perfusion types, and the differences were statistically significant(P<0.05). In the parenchymal stage, the patients in both groups showed mainly uneven enhancement. The differences in the shape, boundary and perfusion characteristics of the perfusion defect area in the two groups were statistically significant(P<0.05). Conclusion: Contrast-enhanced ultrasound is of great value in the differential diagnosis of tuberculous lymphadenitis and metastatic lymph nodes.
