摘 要:目的对比人工耳蜗植入者和非人工耳蜗植入者宽频声导抗测试声能吸收率(wideband absorbance,WBA)的差异,探讨人工耳蜗植入者的宽频声导抗测试声能吸收率的特征。方法选取12例(12耳,男女各6耳)无外中耳疾病史的重度或极重度感音神经性聋行人工耳蜗植入术后一个月的幼儿为实验组,14例(28耳,男女各14耳)无外中耳疾病史的重度或极重度感音神经性聋非人工耳蜗植入幼儿为对照组,分别进行宽频声导抗声能吸收率测试,分析两组结果。结果实验组与对照组之间WBA在低频段(749、1000 Hz)以及高频段(8000 Hz)存在明显差异,实验组的WBA在低频段明显低于对照组,而在高频段高于对照组(P<0.05);对照组在226 Hz男性的WBA明显高于女性,6924 Hz男性的WBA低于女性(均为P<0.05),左右耳之间无统计学差异(P>0.05);实验组在500、749、1000、4896 Hz处,植入耳(右耳)的WBA值低于非植入耳(左耳)(P<0.05),其余频率无明显差异(P>0.05)。结论人工耳蜗植入耳与非植入耳之间WBA存在差异,人工耳蜗植入耳WBA在低频段降低,提示人工耳蜗植入术后中耳系统有改变。Objective To compare the differences of wideband absorbance(WBA)between cochlear implants and non-cochlear implants,and to explore the characteristics of wideband absorbance of cochlear implanters.Methods Twelve infants(12 ears,6 males and 6 males)received cochlear implantation with no history of outer and middle ear diseases were selected as experiment group,and 14 fants(28 ears,14 males and 14 males)with sereve or profund hearing loss without outer middle ear disease were selecoted as control group,and wideband absorbance tests were performed.The statistical differences between the experimental group and the control group,as well as the left and right ear differences and gender differences in the control group were compared.Results There were significant statistical differences between the experimental group and the control group at low frequency(749 Hz,1000 Hz)and high frequency 8000 Hz(P<0.05).The wideband absorbance in the low frequency experimental group was significantly lower than that in the normal group.The wideband absorbance of the high-frequency 8000 Hz experimental group was higher than that of the normal group;there was a statistical difference between the male and female control groups at 226 Hz and 6924 Hz(P<0.05),At 226 Hz,the WBA of males was significantly higher than that of females,and at 6924 Hz,the WBA of males was lower than that of females.There was no statistical difference between the left and right ears(P>0.05).In the experimental group,at 500,749,1000,4896 Hz,the WBA value of the implanted ear(right ear)was lower than that of the non-implanted ear(left ear)(P<0.05),and the other frequencies had no significant difference(P>0.05).Conclusion WBA is different between cochlear implanted ears and non-cochlear implanted ears.WBA in cochlear implanted ears is reduced at low frequency,suggesting that the middle ear system changes after cochlear implantation.WBA has a certain evaluation value for detecting the changes and recovery of the middle ear system after cochlear implantation.
