脑发育异常语前聋患儿人工耳蜗植入疗效分析

摘  要:目的探讨伴脑发育异常的双侧极重度感音神经性语前聋患儿人工耳蜗植入术后的康复效果。方法以2013年11月至2015年9月在中国医科大学附属盛京医院行人工耳蜗植入术的38例脑发育异常(脑异常组)及38例脑发育正常(对照组)的双侧极重度感音神经性语前聋患儿为研究对象,脑异常组植入年龄12.5~138.8月,平均44.5±31.3月;对照组年龄13.8~120.8月,平均42.3±26.4月。两组均行单耳人工耳蜗植入术,术后3~4周开机;于开机后6、12、18、24、30、36个月分别进行声场下听阈测试、听觉行为分级标准(CAP)和言语可懂度分级标准(SIR)评估,比较两组结果。结果开机后6、12、18、24、30、36个月时脑异常组声场下平均听阈与对照组接近,两组间差异无统计学意义(均为P>0.05),两组间CAP、SIR得分差异亦无统计学意义(均为P>0.05)。结论脑发育异常包括多重和严重脑发育异常的重度、极重度感音神经性聋患儿均可行人工耳蜗植入术;本组脑发育异常的双耳极重度感音神经性聋患儿人工耳蜗植入后可获得良好的听觉和言语能力。Objective To investigate the rehabilitation effect of cochlear implantation in patients with bilateral extremely severe sensorineural preverbal deafness and brain dysplasia.Methods 38 deaf children with bilateral extremely severe sensorineural hearing loss and brain dysplasia and 38 deaf children with bilateral extremely severe sensorineural hearing loss and normal brain development who had cochlear implantation in Shengjing Hospital of China Medical University from Nov 2013 to Sep 2015 were included in this study.Children in the observation group were aged from 12.5 to 138.8 months at the time of surgery,with an average age of 44.5±31.3 months.Children in the control group were operated for 13.8~120.8 months,with an average age of 42.3±26.4 months.Both groups underwent monaural cochlear implantation,and the machine was switched on 3~4 weeks after the operation.Subacoustic hearing threshold test after operation was performed at 6,12,18,24,30 and 36 months after start up.And the hearing and speech ability of the deaf children were gradedly evaluated and statistically analyzed by use of CAP and SIR.Results CAP scores of deaf children with brain dysplasia and those without brain dysplasia at 6,12,18,24,30 and 36 months after startup had no statistical difference(P>0.05),and there was no statistical difference in SIR scores at 6,12,18,24,30 and 36 months after startup(P>0.05).Conclusion This study shows that children and bilateral severe sensorineural hearing loss with abnormal brain development can acquire good hearing and speech abilities after cochlear implantation.Cochlear implants are available for children with severe and extremely severe sensorineural hearing loss including multiple and severe abnormalities of brain development.
