双侧侧脑室外引流联合腰大池引流术在重度脑室出血患者中的应用

摘　　要：目的:探讨重度脑室出血患者行双侧侧脑室外引流联合腰大池引流术治疗效果。方法:选取医院收治的76例重度脑室出血患者作为研究对象,按照不同临床治疗方法,分为对照组和实验组,对照组38例患者采取双侧侧脑室外引流术,实验组38例患者采取双侧侧脑室外联合腰大池引流术,比较两组患者脑脊液转清时间、术后7 d血肿清除率、术后并发症、术后3个月格拉斯哥(GOS)预后评分。结果:与对照组相比,实验组患者的脑脊液转清时间缩短,术后7日血肿完全清除率显著增加,术后脑积水发生率显著降低,术后3个月随访发现实验组患者的GOS评分显著提高(P<0.05)。结论:双侧侧脑室外联合腰大池引流术在临床上可有效治疗重度脑室出血,显著降低患者并发症,预后恢复好。Objective:To analyze the treatment effect of bilateral lateral ventricular drainage combined with lumbar cistern drainage in patients with severe ventricular hemorrhage.Methods:76 patients with severe ventricular hemorrhage,according to the different clinical treatment,were divided into control group and experimental group,38 cases of control group adopted bilateral lateral ventricle drainage,38 cases of treatment group adopted add waist beyond the bilateral lateral ventricle drainage pool,compared two groups of cerebrospinal fluid transfer time,7 d of postoperative hematoma clearance rate,postoperative complications,postoperative 3 months Glasgow outcome score(GOS).Results:Compared with the control group,the cerebrospinal fluid clearing time of the experimental group was shortened,the hematoma complete clearance rate was significantly increased,the incidence of postoperative hydrocephalus was significantly decreased,3 months after the operation,the GOS score of the experimental group was significantly increased(P<0.05).Conclusion:Bilateral lateral ventricular drainage combined with lumbar cistern drainage can effectively treat severe ventricular hemorrhage in clinical practice,significantly reduce the complications of patients,and the prognosis is good.
