低温可调钠透析预防透析相关性低血压的观察及护理

摘　　要：目的:观察低温可调钠透析对维持性血液透析(MHD)患者透析相关性低血压的影响。方法:将20例低血压高危透析患者采取治疗前、后作自身对照,每周透析3次,4 h/次,每组治疗模式每周交替进行一次,每例患者一周使用3次常规常温透析(Ⅰ组),另一周采用3次低温可调钠曲线透析(Ⅱ组),两组患者1年内平均各透析1560例次。测量每次透前、透后血压、体温、心率,治疗中每30 min~60 min测量病人心率、血压1次,记录发生低血压例次、临床表现和采取的护理措施。结果:常规常温组(Ⅰ组)和低温可调钠组(Ⅱ组)低血压发生率分别为20.5%和9.9%,Ⅰ组低血压发生率明显高于Ⅱ组(P<0.05)。结论:低温联合可调钠曲线透析方法简单易行、安全可靠,可有效降低发生低血压的概率,使透析平稳渡过,透析充分性明显提高,且无明显的不良反应,值得临床推广应用。Objective:To observe the effect of low-temperature adjustable sodium dialysis curve on dialysis-related hypotension in maintenance hemodialysis(MHD)patients.Methods:20 patients with hypotension high-risk dialysis treatment before and after self-reflection,dialysis three times a week,4 h/times,each treatment group pattern alternates once a week,each patient use 3 times a week regular room temperature dialysis(group I),the other by 3 times a week adjustable low temperature sodium curve dialysis(groupⅡ),two groups of dialysis patients within a year,on average,each 1560 times.Blood pressure,body temperature and heart rate were measured before and after each dialysis.During treatment,heart rate and blood pressure were measured once every 30 min~60 min.The occurrence of hypotension,clinical manifestations and nursing measures were recorded.Results:The incidence of hypotension in the normal temperature group(Group I)and the low-temperature adjustable sodium group(groupⅡ)was 20.5%and 9.9%respectively.The incidence of hypotension in group I was significantly higher than that in groupⅡ.Conclusion:The method of low-temperature combined with adjustable sodium curve dialysis is simple,easy to operate,safe and reliable,and can effectively reduce the probability of hypotension,enable dialysis to pass smoothly,significantly improve the adequacy of dialysis,and have no obvious adverse effects.It is worthy of clinical application.
