老年肿瘤患者采用去白细胞成分输血治疗的临床价值分析

摘　　要：目的分析老年肿瘤患者采用去白细胞成分输血治疗的临床价值。方法96例老年肿瘤患者,根据输血方式不同分为对照组和观察组,每组48例。对照组患者采用全血输血治疗,观察组患者采用去白细胞成分输血治疗。比较两组患者输血前后免疫功能指标、输血量以及不良反应发生情况。结果输血前,两组患者CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)比较差异无统计学意义(P>0.05);输血后,两组患者CD4^(+)、CD4^(+)/CD8^(+)均较输血前降低,CD8^(+)均较输血前升高,但观察组患者CD4^(+)、CD4^(+)/CD8^(+)降低幅度低于对照组,CD8^(+)升高幅度低于对照组,差异均有统计学意义(P<0.05)。对照组患者输血量为(3.47±0.81)U,观察组患者输血量为(3.75±0.86)U,比较差异无统计学意义(t=1.642,P=0.104>0.05)。观察组患者不良反应发生率为6.25%,明显低于对照组的29.17%,差异有统计学意义(P<0.05)。结论老年肿瘤患者采用去白细胞成分输血治疗,相较于全血输血治疗,能够有效降低免疫抑制作用,提高输血治疗安全性,值得临床推广应用。Objective To analyze the clinical value of leukocyte-depleted blood transfusion in elderly patients with tumor.Methods A total of 96 elderly patients with tumor were divided into control group and observation group according to different blood transfusion methods,with 48 cases in each group.The control group was treated with whole blood transfusion,and the observation group was treated with leukocyte-depleted blood transfusion.The immune function indexes before and after blood transfusion,blood transfusion volume and occurrence of adverse reactions were compared between the two groups.Results Before blood transfusion,there was no statistically significant difference in CD4^(+),CD8^(+),CD4^(+)/CD8^(+)between the two groups(P>0.05).After blood transfusion,the CD4^(+)and CD4^(+)/CD8^(+)of the two groups were lower than those before blood transfusion,and CD8^(+)was higher than that before blood transfusion;but the reduction of CD4^(+),CD4^(+)/CD8^(+)of the observation group was lower than that of the control group,and the increase in CD8^(+)was lower than that of the control group.All the difference was statistically significant(P<0.05).The blood transfusion volume of the control group was(3.47±0.81)U,which was(3.75±0.86)U of the observation group,and the difference was not statistically significant(t=1.642,P=0.104>0.05).The incidence of adverse reactions of the observation group was 6.25%,which was obviously lower than 29.17%of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with the whole blood transfusion,leukocyte-depleted blood transfusion can effectively reduce the immunosuppression and improve the safety of blood transfusion of elderly patients with tumor,which is worthy of popularization and application.
