摘 要:目的探讨重症呼吸衰竭患者联合血清N-端脑钠肽前体(NT-proBNP)、高迁移率族蛋白B1(HMGB1)、白介素-17(IL-17)水平与心肌损伤的相关性.方法收集85例呼吸衰竭患者的临床资料,应用电化学发光法检测血清NT-proBNP水平,酶联免疫吸附法检测血清HMGB1水平,放射免疫分析法检测血清IL-17水平,免疫层析定量法检测血清心肌肌钙蛋白I(c Tn-I),分析重症呼吸衰竭患者血清NT-proBNP、HMGB1、IL-17水平表达及与心肌损伤的关系.应用受试者工作特征曲线(ROC曲线)分析血清NT-proBNP、HMGB1、IL-17水平对重症呼吸衰竭患者心肌损伤的预测价值.结果重症呼吸衰竭组血清NT-proBNP、HMGB1、IL-17水平明显高于慢性呼吸衰竭组(P<0.05),心肌损伤组血清NT-proBNP、HMGB1、IL-17水平明显高于心肌正常组(P<0.05),血清NT-proBNP、HMGB1和IL-17是重症呼吸衰竭患者心肌损伤的影响因素(P<0.05).血清NT-proBNP、HMGB1、IL-17水平联合检测诊断重症呼吸衰竭患者心肌损伤的曲线下面积为0.893,敏感度为84.6%,较三者单一检测水平高.结论重症呼吸衰竭患者血清NT-proBNP、HMGB1、IL-17水平呈明显升高趋势,且三者与重症呼吸衰竭患者心肌损伤有关,或可作为预测心肌损伤的有效标志物.Objective To investigate the correlation of serum N-terminal pro-brain natriuretic peptide(NT-proBNP),high mobility group protein B1(HMGB1)and interleukin-17(IL-17)levels with myocardial injury in patients with severe respiratory failure.Method The clinical data of 85 patients with respiratory failure were collected.Electrochemiluminescence assay was used to detect serum NT-proBNP level,enzyme-linked immunosorbent assay was used to detect serum HMGB1 level,radioimmunoassay was used to detect serum IL-17 level,and immunochromatography quantitative method was used to detect serum cardiac troponin I(cTn-I).The relationship of serum NT-proBNP,HMGB1 and IL-17 with myocardial injury in patients with severe respiratory failure was analyzed.Receiver operating characteristic curve(ROC curve)was used to analyze the predictive value of NT-proBNP,HMGB1 and IL-17 for myocardial injury in patients with severe respiratory failure.Results Serum NT-proBNP,HMGB1 and IL-17 levels in the severe respiratory failure group were significantly higher than those in the chronic respiratory failure group(P<0.05).Serum NT-proBNP,HMGB1 and IL-17 levels in the myocardial injury group were significantly higher than those in the normal myocardium group(P<0.05).Logistic analysis indicated that serum NT-proBNP,HMGB1 and IL-17 were the influencing factors of myocardial injury in patients with severe respiratory failure(P<0.05).The area under the curve(AUC)and sensitivity of serum NT-proBNP combined with HMGB1 and IL-17 for diagnosing myocardial injury in patients with severe respiratory failure were 0.893 and 84.6%,higher than those of the three alone.Conclusion Serum NT-proBNP,HMGB1 and IL-17 levels significantly elevated in patients with severe respiratory failure,and they are related to myocardial injury,which indicates that they may be used as effective markers for predicting myocardial injury.
