摘 要:目的探讨人类白细胞抗原-G对儿童喘息性疾病的影响及作用机制。方法选取2018年6月至2019年7月收治的喘息性疾病患者90例,根据患儿是否具有喘息症状和哮喘预测指数(API)是否为阳性分为3组,其中喘息+API(+)为A组,喘息+API(-)为B组,非喘息的下呼吸道感染为C组,再根据呼吸道病毒是否为阳性分为阳性组和阴性组,选取同时期就诊且呼吸系统正常者30例作为对照组,检查所有研究对象可溶性人类白细胞抗原G(sHLA-G)、白细胞介素(IL)-4、IL-10和γ-干扰素(IFN-γ)水平,使用SPSS25.0统计软件进行分析。结果A组血清sHLA-G、IFN-γ和IL-10水平最低,其次是B组,4组比较,差异有统计学意义(P<0.05),且相邻两组间比较,差异均有统计学意义(P<0.05)。A组IL-4水平最高,其次是B组,4组比较,差异有统计学意义(P<0.05),且相邻两组间比较,差异均有统计学意义(P<0.05)。呼吸道病毒结果显示,阳性患者有25例,阴性35例,阳性组血清sHLA-G、IL-4、IL-10、IFN-γ水平与阴性组和对照组比较,差异有统计学意义(P<0.05),3组比较差异有统计学意义(P<0.05);阳性组血清sHLA-G、IFN-γ和IL-10水平最低,IL-4水平最高。结论呼吸道病毒感染和喘息性疾病的患者血清sHLA-G、IFN-γ和IL-10水平降低,IL-4水平升高,四者可能共同参与儿童喘息性疾病的气道炎性反应,对临床诊治和预后具有重要意义。Objective To investigate the effect of human leukocyte antigen G(HLA-G)on asthmatic diseases in children and its mechanism.Methods A total of 90 asthmatic patients who were admitted from June 2018 to July 2019 were divided into three groups according to whether they had asthmatic symptoms and whether they had positive asthmatic predictive index(API),wheezing+API(+)was group A,wheezing+API(-)was group B,and non wheezing lower respiratory tract infection was group C,and were divided into positive group and negative group according to whether the respiratory virus was positive or not.A total of 30 patients with normal respiratory system were selected as the control group.The levels of soluble human leukocyte antigen G(sHLA-G),interleukin(IL)-4,IL-10 andγ-Interferon(IFN-γ)in all subjects were examined,and were analyzed by SPSS25.0 statistical software.Results The serum levels of sHLA-G,IFN-γand IL-10 in group A were the lowest,followed by group B.There was significant difference among the four groups(P<0.05).The difference between the two adjacent groups was statistically significant(P<0.05).The level of IL-4 in group A was the highest,followed by group B.the difference was statistically significant(P<0.05).The difference between the two adjacent groups was statistically significant(P<0.05).The results of respiratory virus test showed that 25 patients were positive and 35 were negative.The expression levels of serum sHLA-G,IL-4,IL-10 and IFN-γin the positive group were significantly different from those in the negative group and the control group(P<0.05).The difference among three groups was statistically significant(P<0.05).The levels of serum sHLA-G,IFN-γand IL-10 in the positive group were the lowest and IL-4 level was the highest.Conclusion The levels of serum sHLA-G,IFN-γand IL-10 in patients with respiratory virus infection and asthmatic diseases decrease,while the level of IL-4 increases.The four indexes might participate in the airway inflammatory response of children with asthmatic diseases,which is of
