摘　　要：目的:探究血液灌流联合左卡尼汀治疗对维持性血液透析(MHD)患者的疗效分析。方法:将100例MHD患者按照治疗方案不同分为实验组和对照组,每组各50例;对照组接受常规透析治疗和血液灌流治疗,实验组在对照组的基础上加载左卡尼汀治疗。6个月后,比较两组患者营养状况[血红蛋白(Hb)、血清白蛋白(Alb)、前白蛋白(PA)]、睡眠质量、微炎症[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(Hs-CRP)]状态以及生活质量。结果:治疗6个月后,实验组患者Hb、Alb、PA水平均高于对照组(P<0.05),TNF-α、IL-6、Hs-CRP水平均低于对照组(P<0.05),匹兹堡睡眠质量指数(PSQI)量表各项评分均低于对照组(P<0.05),生活质量评价量表(SF-36)各项评分均高于对照组(P<0.05)。结论:血液灌流联合左卡尼汀治疗能提高MHD患者的营养水平、睡眠质量和生活质量,降低患者的微炎症水平。Objective:To explore the curative effect of hemoperfusion combined with L-carnitine on patients with maintenance hemodialysis(MHD).Methods:100 MHD patients were selected as the research objects.According to the treatment plan,they were numbered,and then they were randomly divided into experimental group and control group,with 50 cases in each group.The control group received routine dialysis treatment and hemoperfusion treatment,and the experimental group received L-carnitine treatment on the basis of the control group.6 months later,the nutritional status[hemoglobin(Hb),serum albumin(Alb),prealbumin(PA)],sleep quality,microinflammation[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),high-sensitivity C-reactive protein(Hs-CRP)]status and quality of life were compared between the two groups.Results:After 6 months of treatment,the levels of Hb,Alb and PA in the experimental group were higher than those in the control group(P<0.05),the levels of TNF-α,IL-6 and Hs-CRP were lower than those in the control group(P<0.05).The scores of Pittsburgh Sleep Quality Index(PSQI)were lower than those in the control group(P<0.05),and the scores of SF-36 were higher than those in the control group(P<0.05).Conclusion:Hemoperfusion combined with L-carnitine therapy can improve the nutrition level,sleep quality and quality of life of MHD patients and reduce the microinflammation level of patients.
