血清miR-144水平在膀胱癌诊断及预后评价中的价值分析

摘  要:目的:分析血清微小RNA(miR)-144水平在膀胱癌诊断及预后评价中的价值。方法:选取手术治疗的96例膀胱癌患者作为病例组,选取同期100名健康体检者作为对照组。采用实时聚合酶链式反应(RT-PCR)法对两组研究对象血清miR-144水平进行检测和对比,并分析不同临床特征膀胱癌患者血清miR-144水平的差异,采用受试者工作特征(ROC)曲线针对血清miR-144水平在膀胱癌诊断中的价值进行分析;对患者进行为期36个月的术后随访,对其总生存期(OS)和无进展生存期(PFS)进行随访,并分析影响患者预后的相关因素。结果:病例组患者的血清miR-144水平低于对照组,肿瘤分期T_(2)~T_(4)患者的血清miR-144水平低于肿瘤分期T_(0)~T_(1)患者,有淋巴结转移患者的血清miR-144水平均低于无淋巴结转移患者(P<0.05)。血清miR-144水平在诊断膀胱癌、T_(2)~T_(4)期膀胱癌、膀胱癌淋巴结转移中的ROC曲线下面积(AUC)均具有统计学意义(P<0.05),其中在诊断膀胱癌中的AUC最高,为0.896。Kaplan-Meier生存分析结果显示,低血清miR-144水平组患者的OS、PFS估计值均低于高miR-144水平组(P<0.05)。Cox风险回归模型分析结果显示,膀胱癌患者术后OS和PFS与血清miR-144水平和肿瘤类型均具有相关性(P<0.05),其中肌层浸润性膀胱癌(MIBC)可提升患者不良预后风险,高血清miR-144水平可降低患者不良预后风险。结论:膀胱癌患者表现为血清miR-144水平的降低,其水平与患者的预后具有相关性,可用于辅助膀胱癌的早期诊断和预后评价。Objective:To analyze the values of serum microRNA(miR)-144 level in the diagnosis and prognosis evaluation of bladder cancer.Methods:96 patients with bladder cancer who underwent surgical treatment were selected as the case group,and 100 healthy people were selected as the control group.The serum miR-144 levels of the subjects in the two groups were detected and compared by real-time polymerase chain reaction(RT-PCR),and the differences of serum miR-144 levels in patients with bladder cancer with different clinical characteristics were analyzed.Receiver operating characteristic(ROC)curve was employed to analyze the value of serum miR-144 level in the diagnosis of bladder cancer,the patients were followed up for 36 months,their overall survival(OS)and progression free survival(PFS)were followed up,and the related factors affecting the prognosis of patients were analyzed.Results:The serum miR-144 level of the patients in the case group was lower than that in control group,the serum miR-144 level of the patients with tumor stage T_(2)-T_(4) was lower than that of the patients with tumor stage T a~T_(1),and the serum miR-144 level of the patients with lymph node metastasis was lower than that of the patients without lymph node metastasis,the differences were statistically significant(P<0.05).The area under the ROC curve(AUC ROC)of serum miR-144 level in the diagnosis of bladder cancer,T_(2)-T_(4) bladder cancer and lymph node metastasis of bladder cancer was statistically significant(P<0.05),among them,AUC ROC was the highest in the diagnosis of bladder cancer,which was 0.896.Kaplan Meier survival analysis showed that the OS and PFS estimates of the patients with low serum miR-144 level were lower than those of the patients with high miR-144 level,the differences were statistically significant(P<0.05).Cox risk regression model analysis results showed OS and PFS of bladder cancer patients after operation were correlated with serum miR-144 level and tumor type(P<0.05),the incidence of muscle invasive bladder cancer(MIB
