基于自我效能的护理对乳腺癌根治术患者病耻感及患肢水肿的影响

摘　　要：目的:探讨基于自我效能的护理对乳腺癌根治术患者病耻感及患肢水肿的影响。方法:76例乳腺癌根治术患者按护理方式不同分为对照组和观察组,每组各38例。对照组采用常规护理,观察组在对照组的基础上采用基于自我效能的护理。比较两组患者的病耻感、功能锻炼的依从性、肩关节功能及患肢水肿情况。结果:观察组的社会排斥、经济歧视、内在羞耻感等社会影响量表(SIS)评分低于对照组(P<0.05),锻炼依从性高于对照组(P<0.05),肩关节功能好于对照组(P<0.05),患肢水肿发生率低于对照组(P<0.05)。结论:基于自我效能的护理能够减轻乳腺癌根治术患者术后的病耻感,提高患肢锻炼依从性,改善患肢功能,减少患肢水肿的发生。Objective:To explore the effect of self-efficacy nursing on stigma and limb edema of patients with radical mastectomy.Methods:76 cases of breast cancer patients were divided into control group and observation group according to different nursing methods,38 cases in each group.The control group was given routine nursing,and the observation group was given self-efficacy nursing on the basis of the control group.The shame,compliance of functional exercise,shoulder joint function and edema of affected limb were compared between the two groups.Results:After the intervention,the SIS scores of social exclusion,economic discrimination and internal shame in the observation group were lower than those in the control group(P<0.05).The exercise compliance of the observation group was higher than that of the control group(P<0.05),the shoulder joint function of the intervention group was better than that of the control group(P<0.05),the edema rate of the affected limb was lower than that of the control group(P<0.05).Conclusion:Nursing based on self-efficacy can reduce the stigma of patients after radical mastectomy,improve the exercise compliance of affected limb,improve the function of affected limb,and reduce the occurrence of edema of affected limb.
