超声造影与彩色多普勒超声在肝实质性肿块诊断中的临床应用

摘　　要：目的:探析超声造影与彩色多普勒超声在肝实质性肿块诊断中的临床应用价值。方法:选取本院肝实质性肿块患者70例(2018年2月~2019年4月)为研究对象,所有研究对象均进行超声造影与彩色多普勒超声检查,比较两种超声检查方法的病灶检出率。结果:病理组织学证实,70例患者中共检出肝实质性肿块90个,其中,恶性肿瘤53个(原发性肝癌35个、肝转移癌18个),良性肿瘤为肝血管瘤22个,良性病变15个,肝局灶性结节增生9个,肝孤立性坏死结节6个。超声造影的病灶检出率为:原发性肝癌85.71%,肝转移癌77.78%,肝血管瘤81.82%,肝局灶性结节增生77.78%,肝孤立性坏死结节83.33%,均高于彩色多普勒超声的检出率,P<0.05。结论:在肝实质性肿块诊断中,超声造影与彩色多普勒超声均能进行有效诊断,但是超声造影优势更加明显,能为临床诊断提供影像学参考。Objective:To explore the clinical value of contrast-enhanced ultrasound and color Doppler ultrasound in the diagnosis of liver parenchymal masses.Methods:70 patients(February 2018 to April 2019)with liver parenchymal mass in our hospital were selected as the study objects.All the subjects were examined by contrast-enhanced ultrasound and color Doppler ultrasound,and the detection rate of lesions was compared between the two methods.Results:Histopathology confirmed that 90 solid masses of liver were detected in 70 patients,including 53 malignant tumors,35 primary liver cancer,18 metastatic liver cancer,22 benign hemangiomas,15 benign lesions,9 focal nodular hyperplasia and 6 solitary dead nodules.The detection rate of focus in contrast-enhanced ultrasound was 85.71%in primary liver cancer,77.78%in metastatic liver cancer,81.82%in hemangioma,77.78%in focal nodular hyperplasia and 83.33%in solitary necrotic nodule of liver,all higher than that in color Doppler ultrasound(P<0.05).Conclusion:In the diagnosis of liver parenchymal mass,both contrast-enhanced ultrasound and color Doppler ultrasound can be used for effective diagnosis,but contrast-enhanced ultrasound has more obvious advantages,which can provide imaging reference for clinical diagnosis.
