摘  要:目的:探究乳糜血样对血液分析仪测定血红蛋白结果的影响及消除效果。方法:在2018年12月~2019年12月时间段中,选取90例健康体检者作为研究主体,将其分为A组、B组与C组,每组30例,分别将血浆替换成等量的脂肪乳25μL、50μL、75μL,将其模拟成不同程度的乳糜标本,检测血液中脂肪乳对血红蛋白造成的影响。同期选取输入脂肪乳体检者30例作为研究对象,取患者的乳糜血样,血浆置换等量生理盐水,对影响到血液中血红蛋白的乳糜情况进行消除。结果:A组、B组与C组脂肪乳置换后与置换前,血红蛋白检测结果存在明显差异,有统计学意义,P<0.05。乳糜血样直接测定血红蛋白检测水平与置换生理盐水后血红蛋白检测水平存在显著差异,P<0.05。结论:在血液分析仪检测中,乳糜血样中的血红蛋白假性情况明显增高,随着脂肪乳含量不断增加,对血液中血红蛋白造成的影响程度也会随之增加。通过采用生理盐水等量置换血浆后,能够有效地消除乳糜血样对血红蛋白造成的影响,可以提高血红蛋白检测的准确性。Objective:To investigate the effect of chylous blood sample on the determination of hemoglobin by hematology analyzer and its elimination effect.Methods:During the period from December 2018 to December,2019,90 healthy subjects were selected as the subjects of the study,and they were divided into group A,group B and group C,with 30 patients in each group.The plasma was replaced with 25,50 and 75 micrograms of the same amount of fat milk,respectively,and the samples were simulated into fascinating samples of different degrees to detect the effect of fat milk in the blood on hemoglobin.During the same period,30 patients who underwent physical examination with fat milk were selected as the study objects.Chylous blood samples of the patients were taken and plasma exchange was performed with the same amount of normal saline to eliminate chylous conditions affecting hemoglobin in the blood.Results:The hemoglobin test results of group A,group B and group C were significantly different after and before fat milk replacement,with statistical significance(P<0.05).There was a significant difference between the hemoglobin level detected by chylous blood sample directly and that after the replacement of normal saline(P<0.05).Conclusion:Hemoglobin pseudoactivity in chylous blood samples was significantly increased by hematology analyzer,and the effect on hemoglobin in blood increased with the increase in fat milk content.The effect of chylous blood sample on hemoglobin can be effectively eliminated by replacing plasma with normal saline,which can improve the accuracy of hemoglobin detection.
