摘 要:目的探讨常规超声联合高频超声在小儿肠系膜淋巴结增大诊断中的应用价值。方法研究以方便选取该院2017年8月—2019年8月收治的126例肠系膜淋巴结增大患儿为诊断对象,全部患者均使用常规超声检查和高频超声检查,将常规超声检查结果纳入对照组(n=126),将常规超声联合高频超声检查结果纳入观察组(n=126),比较两组肠系膜淋巴结增大结果,并选取到该院进行体检的126名健康儿童作为健康组,分析小儿肠系膜淋巴结增大影像学表现。结果观察组诊断正确率(99.21%)明显高于对照组(86.51%),组间差异有统计学意义(χ^(2)=15.316,P<0.05)。小儿肠系膜淋巴结增大影像学表现为全部患儿均诊断为多发淋巴结增大,一般数量为2~5个,增大的淋巴结数量主要位于右中下腹,仅有少数位于左中上腹,诊断影像中显示,淋巴结为长椭圆形,可见低回声包块,且内部回声均匀,皮髓质分界明显、光滑,并且有增大现象,长短径比≥2,增大淋巴结中可见稀疏或丰富树枝条状、点状血流信号,以淋巴门型方式分布,少见液性暗区。结论对肠系膜淋巴结增大患儿采用常规超声联合高频超声检查,能够更好地对患儿进行疾病诊断,获取肠系膜淋巴结增大病情特征,将其与阑尾炎等区分开来,相比于仅实施常规超声诊断方式,联合诊断方式诊断结果更优,诊断结果可信度更高,可为患儿病情诊断提供可靠参考,具有更高应用价值。Objective To explore the application value of conventional ultrasound combined with high-frequency ultrasound in the diagnosis of mesenteric lymph node enlargement in children.Methods Selected conveniently 126 children with mesenteric lymph node enlargement admitted to the hospital from August 2017 to August 2019 as the diagnostic object.All patients used conventional ultrasound and high-frequency ultrasound,and the results of conventional ultrasound were included in the control group(n=126),the results of conventional ultrasound combined with high-frequency ultrasound were included in the observation group(n=126),the results of mesenteric lymph node enlargement between the two groups were compared,and 126 healthy children who were examined in the hospital were selected as the healthy group.and the children were analyzed imaging findings of enlarged mesenteric lymph nodes.Results The diagnostic accuracy rate of the observation group(99.21%)was significantly higher than that of the control group(86.51%),and the difference between the groups was statistically significant(χ^(2)=15.316,P<0.05).The imaging manifestation of enlarged mesenteric lymph nodes in children was that all children were diagnosed with multiple enlarged lymph nodes,usually 2-5.The number of enlarged lymph nodes was mainly located in the right middle and lower abdomen,and only a few were located in the left middle and upper abdomen.Diagnostic imaging showed that the lymph node was oblong,with hypoechoic masses,and the internal echo was uniform,the cortex and medulla boundary was obvious,smooth,and enlarged,the ratio of length to diameter was≥2,and the enlarged lymph node can be sparse or rich in branches strip and punctate blood flow signals were distributed in a lymphatic portal pattern,and liquid dark areas were rare.Conclusion The use of conventional ultrasound combined with high-frequency ultrasound for children with enlarged mesenteric lymph nodes can better diagnose the disease and obtain the characteristics of the disease of the enlarged
