摘  要:目的分析有麻疹疫苗接种史麻疹病例的麻疹病毒血凝素(Hemagglutinin,HA)基因变异特征。方法筛选2015-2018年河北省有麻疹疫苗接种史病例的麻疹病毒分离株,通过逆转录-聚合酶链反应(RT-PCR)扩增HA全基因并测序,分析HA基因核苷酸和氨基酸序列同源性和氨基酸位点变异。结果共获得16株符合条件的麻疹病毒分离株,均为H1a基因亚型,其与中国沪191疫苗株之间HA基因核苷酸和氨基酸序列的同源性分别为94.01%-94.82%和94.82%-95.47%。所有分离株发生S240N或S240D突变,导致潜在糖基化位点NLS238-240缺失。所有分离株的CD46受体结合位点发生T481N突变,14株分离株的中和位点发生P397L突变。与河北省2005-2008年流行麻疹毒株相比,所有分离株存在1个共同的新突变位点(Q334H),14株分离株存在3个共同的新突变位点(D574A、G592R和G603E);与2015-2017年无麻疹疫苗接种史病例的流行代表株相比无共同新突变。结论麻疹病毒HA存在抗原漂移现象,但主要免疫优势表位未发生明显变化;需持续监测麻疹病毒HA基因变异。Objective To analyze variation of hemagglutinins(HA)of measles virus(MV)isolates from vaccinated measles cases.Methods We selected MV strains isolated from measles cases who had received measles vaccine between 2015 and 2018 in Hebei province;we amplified whole HA genes by reverse transcription-polymerase chain reaction(RT-PCR)to determine nucleotide sequences;we quantified HA gene nucleotide homologies and HA amino acid homologies.Results Sixteen MV strains were obtained and identified as H1 a genotype.HA nucleotide and amino acid homologies between the isolates and the China measles vaccine strain S191 were 94.01%-94.82%and 94.82%-95.47%,respectively.All isolates had the S240 N or N240 D mutation,resulting in potential loss of the N-glycolyzation site NLS238-240.All isolates had the T481 N mutation at the receptor binding site of CD46,and neutralizing sites of 14 isolates had the P397 L mutation.Compared with MV strains circulating in Hebei during 2005-2008,all isolates had a new common mutation site(Q334 H),and 14 isolates had 3 new common mutation sites(D574 A,G592 R and G603 E).No new common mutation sites were found compared with reference strains isolated from measles cases who had not received measles vaccine during 2015-2017 in Hebei.Conclusions The HA of MVs showed antigenic drifts but no significant variations were found in its major immunodominant epitopes.We need to continuously monitor variations of HA genes of MVs.
