快速康复外科在二期小儿尿道下裂治疗中的应用

摘　　要：目的探讨快速康复外科(ERAS)在小儿尿道下裂二期手术应用的可行性。方法选择2018年1月至2019年12月在我院已行一期阴茎下曲矫正且尿道外口位于阴茎体部的尿道下裂患儿62例,根据家长和患儿的意愿分为两组,ERAS组31例,传统组31例。比较两组患儿术后并发症、住院时间、住院费用等。结果ERAS组术后尿瘘1例,占3.23%,无其他并发症;传统组术后泌尿系感染3例,占9.68%,无其他并发症,两组并发症总发生率比较,差异无统计学意义(P>0.05)。ERAS组术后住院时间、住院费用均低于传统组,差异有统计学意义(P<0.05)。结论ERAS应用于已矫正阴茎下曲的二期尿道下裂患儿手术是安全、可行的,能缩短住院时间,节约医疗成本。Objective To investigate the feasibility of application of enhanced recovery after surgery(ERAS)in the operation of phaseⅡhypospadias in children.Methods A total of 62 children with hypospadias who had undergone correction of phaseⅠhypospadias and whose external urethral orifice was located in the body of penis admitted to our hospital from January 2018 to December 2019 were selected and divided into the ERAS group(n=31)and the conventional grou p(n=31)according to the will of parents and children.The inciden ce of postoperative complications,hospitalization time and hospitalization expenses were compared between the two groups of children.Results In the ERAS group,there was 1 case of postoperative urinary fistula,accounting for 3.23%,without other complications.While in the conventional group,there were 3 cases of postoperative urinary tract infection,accounting for 9.68%,without other complications.There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).The postoperative hospitalization time and hospitalization expenses in the ERAS group were both lower than those in the conventional group,with statistically significant differences(P<0.05).Conclusion ERAS is safe and feasible in the treatment of children who have undergone correction of phaseⅡhypospadias.And it can shorten the hospitalization time and sa ve the medical expenses.
