纤维支气管镜引导下双腔支气管导管插管在胸外科手术麻醉中的应用效果

摘　　要：目的探讨纤维支气管镜引导下行双腔支气管导管插管在胸外科手术麻醉中的临床应用可行性。方法本研究选取了2018年10月至2019年6月前来本院进行疾病检查及治疗的患者为对象,经过专业的科室检查需要开展胸外科手术治疗。在自愿参与试验调查的患者中,将48例患者硬币随机法分为两组:对照组患者(n=24)应用传统的听诊方式;观察组患者(n=24)应用纤维支气管镜,并记录所有患者的心率、平均动脉压情况。结果观察组患者的准确定位率为95.83%(23/24)明显高于对照组为54.17%(13/24)(P<0.05);观察组患者在平均定位所需的时间为(63.40±18.70)s明显低于对照组患者(82.30±21.50)s(P<0.05);对照组患者有2例出现气道损伤,观察组无患者发生此情况,表明观察组的操作更为安全有效;在插管完成3 min后,对照组患者心率和平均动脉压水平分别为(76.02±12.05)次/分和(88.80±12.60)mm Hg与观察组患者(75.01±11.07)次/分和(88.26±13.61)mm Hg相比,均无明显统计学差异(均P>0.05)。结论纤维支气管镜引导下双腔支气管导管插管对于胸外科手术麻醉具有重要作用,有利于提高定位的准确率,并减少所需时间,从而提升治疗成功率。Objective This experiment will adopt the strategy of double-cavity bronchial catheter intubation guided by fiberoptic bronchoscopy,and apply it to the thoracic surgery to analyze the clinical effect.Methods This experiment selected patients who came to the hospital for disease examination and treatment from October 2018 to June 2019.After professional examination,they needed to undergo thoracic surgery.Among the patients who volunteered to participate in the experimental survey,the results of 48 patients were discussed by using the coin randomization method.The control group(n=24)adopted the traditional stethoscope,while the observation group(n=24)had fiberoptic bronchoscopy,and recorded the heart rate and average arterial pressure.Results From the specific situation,the accurate location rate of patients in the observation group was 95.83%(23/24),and the control group was 54.17%(13/24),and the difference between groups was significant and statistically significant(P<0.05).At the same time,the observation group was(63.40±18.70)s in the time required for average positioning,and(82.30±21.50)s in the control group,with large differences between groups.In addition,we also looked at the airway injury of the patients,that is,there were 2 cases in the control group,but this problem did not occur in the observation group,so that the observation group's operation was safer and more effective.Finally,in the comparison of the patient's heart rate and average arterial pressure,the patient's difference at each point in time is not significant,that is,3 min after the intubation is completed,the heart rate and average arterial pressure levels in the control group were(76.02±12.05)times/min and(88.80±12.60)mm Hg,while the observation group was(75.01±11.07)times/min and(88.26±13.61)mm Hg,there was no significant statistical difference(all P>0.05).Conclusion Using fiberoptic bronchoscopy to guide double-cavity bronchial catheter intubation plays an important role in thoracic surgery anesthesia,which is beneficial to improve
