奥曲肽治疗肝硬化合并上消化道出血的临床疗效观察

摘　　要：目的研究奥曲肽治疗肝硬化合并上消化道出血的临床效果。方法将2018年3月至2019年6月到我院治疗的90例肝硬化合并上消化道出血的患者随机分为观察组和对照组,每组45例。两组患者均进行常规治疗,对照组配合应用泮托拉唑;观察组配合用药奥曲肽,比较两组患者的治疗效果。结果观察组病情控制率为95.56%(43/45),对照组病情控制率为73.33%(33/45),P<0.05;观察组再出血发生率为6.98%(3/43),对照组在出血发生率为21.21%(7/33),P<0.05;观察组并发症发生率为8.89%(4/45),对照组为26.67%(12/45),P<0.05;观察组的止血时间为(19.24±3.82)h,对照组为(28.46±4.15)h,P<0.05,存在统计学差异性。结论在常规治疗基础上使用奥曲肽,可有效提升对肝硬化合并上消化道出血的病情控制率,降低患者再出血的发生率和并发症的发生率,缩短患者的止血时间,值得推广。Objective To study the clinical effect of octreotide in the treatment of liver cirrhosis with upper gastrointestinal bleeding.Methods Ninety patients with liver cirrhosis and upper gastrointestinal bleeding who were treated in our hospital from March 2018 to June 2019 were randomly divided into observation group and control group with 45 cases.Both groups of patients received conventional treatment,the control group was combined with pantoprazole;the observation group was combined with octreotide,and the treatment effects of the two groups were compared.Results The disease control rate of the observation group was 95.56%(43/45),and the control rate of the control group was 73.33%(33/45),P<0.05.The incidence of rebleeding in the observation group was 6.98%(3/43).The incidence of bleeding was 21.21%(7/33),P<0.05;the incidence of complications in the observation group was 8.89%(4/45),and that in the control group was 26.67%(12/45),P<0.05;the write-only time was(19.24±3.82)h,and the control group was(28.46±4.15)h,P<0.05,and there was statistical difference.Conclusion On the basis of conventional treatment,esoteric peptide can effectively improve the control rate of cirrhosis with upper gastrointestinal hemorrhage,reduce the incidence of rebleeding and the incidence of complications,and shorten the hemostasis time of patients.It is worthy of promotion.
