来氟米特和环磷酰胺分别联合激素用于难治性肾病综合征治疗的疗效比较

摘　　要：目的分析并比较来氟米特联合激素与环磷酰胺联合激素对难治性肾病综合征的治疗效果。方法选取2017年2月至2019年5月入院接受治疗的难治性肾病综合征患者80例进行分组治疗,采用数字表法进行分组,每组40例。对照组采用环磷酰胺与激素联合治疗,观察组在采取来氟米特与激素联合治疗。治疗后对所得疗效进行比较。结果两组患者的总有效率比较,观察组为97.50%,对照组为85.00%,数据有统计学差异(P<0.05)。观察组与对照组治疗前各项临床指标比较,差异不明显(P>0.05)。治疗后观察组患者的24 h尿蛋白定量为(1.21±0.85)g/24 h、三酰甘油为(1.21±0.32)mmol/L、胆固醇为(5.81±0.93)mmol/L以及白蛋白为(35.33±2.27)g/L,均优于对照组的(2.39±0.87)g/24 h、(1.38±0.28)mmol/L、(4.40±0.79)mmol/L、(30.08±2.22)g/L,数据有统计学差异(P<0.05)。结论与环磷酰胺联合激素相比,来氟米特联合激素治疗难治性肾病综合征的效果更为显著,有利于进一步提高治疗效果,改善生化指标,应用价值较高。Objective To analyze and compare the therapeutic effects of leflunomide combined with hormone and cyclophosphamide combined with hormone on refractory nephrotic syndrome.Methods A total of 80 patients with refractory nephrotic syndrome admitted to hospital for treatment from February 2017 to May 2019 were divided into groups by digital table method,with 40 cases in each group.The control group was treated with cyclophosphamide combined with hormone.The observation group was treated with leflunomide combined with hormone.The therapeutic effects were compared after treatment.Results Comparing the total effective rate of the two groups of patients,the observation group was 97.50%and the control group was 85.00%,and the data was statistically different(P<0.05).There was no significant difference between the observation group and the control group in various clinical indicators before treatment(P>0.05).After treatment,the 24 h urine protein quantitative of the observation group was(1.21±0.85)g/24 h,triacylglycerol was(1.21±0.32)mmol/L,cholesterol was(5.81±0.93)mmol/L,and albumin was(35.33±2.27)g/L,which were better than those of the control group(2.39±0.87)g/24 h,(1.38±0.28)mmol/L,(4.40±0.79)mmol/L,(30.08±2.22)g/L,the data were statistically different(P<0.05).Conclusion Compared with cyclophosphamide combined with hormone,leflunomide combined with hormone is more effective in the treatment of refractory nephrotic syndrome,which is conducive to further improving the treatment effect and biochemical indicators.
